FACILITY OVERVIEW:
Davie County operates a conventional water treatment plant with a discharge of wastewater generated during
the flocculation and sedimentation process in upflow clarifiers and from clarifier and filter backwash. This
water treatment plant currently has a design potable flowrate of 3.0 MGD and a maximum, monthly average
wastewater discharge of 1.38 MGD (based on the last three years of DMR data). The WTP draws raw water from
the Yadkin River. This facility is located at the Sparks Road WTP at 728 Sparks Road, Mocksville, in Davie
County.

Raw Water Treatment Process: (from application and historical documents)
The water treatment plant draws water from Yadkin River adding activated carbon slurry (ACS) and liquid
copper sulfate as it is pumped to a 15 MG raw water reservoir. From the reservoir raw water has addition of
more ACS, caustic, ferric sulfate and chlorine which is mixed in an inline mixer and then separated across two
(2) upflow clarifiers. Settled water from the clarifiers has chlorine and caustic added and pass through four (4)
multi -media filters. Filtered water then has orthophosphate, caustic, fluoride and chlorine added before going
to a clearwell prior to being pumped to the distribution system. Media in the filters consists of coal/anthracite;
intermediate sand; high density sand; high density support gravel; silica gravel.
Wastewater and Treatment Process: (from application, and historical documents)
Wastewater flows to two settling lagoons, each 0.5 acres in surface area. The wastewater is generated from
waste flow from the clarifiers and the filters as well as backwash from the clarifiers and filters. Decant from the
settling lagoons is dechlorinated with sodium sulfite before discharge to a UT to the Yadkin River. Sludge in the
settling lagoons is removed every 6-8 years and land applied under permit WQ0028806.
APPLICATION
The Water Treatment Plant is operated by the Davie County Water System. The application outlines a
treatment process as outlined above utilizing flocculation, sedimentation (upflow clarifiers), and filtration
(conventional multi -media) to produce potable water with wastewater and sediments going to two settling
lagoons. Raw water is noted as coming from the Yadkin River. The application notes average flow from the
lagoons being 0.52 MGD. The application requests waiver for Table A parameters: BOD, COD, total organic
carbon, NH3-N and temperature. These parameters are not required by the existing permit and do not appear to
be of concern, so the waiver is approved. The application notes the following parameters as "Believed Present"
Cu, Zn, TRC, fluoride, TN, TP and Mn. Of these all are monitored in the current permit.
The application notes that sludge depth as settled in the lagoons is monitored bi-annually and solids are
removed approximately every 6 - 8 years for land application under a Davie County non -discharge permit -
WQ0028806.
In the application and in BIMS the facility coordinates were given as 36' 02' 34" / 80' 29' 28". These
coordinates seem to actually show the location where the UT meets the Yadkin River. Based on the google map
of the facility, the facility location coordinates in BIMS have been changed to 36' 01' 58" / 80' 30' 01".
INSPECTION
The most recent inspection occurred on January 12, 2023. The inspection report did not note any major issues
and indicated the facility was well maintained, etc.

COMPLIANCE:
There are no enforcement actions noted in BIMS over the lifetime of this permit. The facility had two (2) NOVs
during the permit cycle, one for a TSS violation and one for frequency violations for Cu and Zn monitoring. The
facility also had one NOD for a frequency violation for hardness monitoring.
MONITORING DATA REVIEW:
The data analysis below is based on all DMR data since the last permit renewal (from December 2019 to
February 2024). Only the last three years of data (March 2021 - February 2024) were used in setting the
maximum monthly flow and as input for the RPA analysis.

The data reviewed for the renewal is from December 2019 through February 2024.
REVIEW OF PERMIT MONITORING AND LIMITS:
Flow - The maximum monthly average flow from the last three years of DMRs (March 2021 - Feb 2024) was
1.38 MGD. Flows from this facility over the course of the permit decreased considerably beginning around July
of 2021. From December 2019 through July 2021 the average flow was 1.21MGD and from August 2021
through February 2024 the average flow was 0.30MGD. The flow was essentially cut to one-fourth of its
previous rate. The county was asked about this flow reduction in an email add -info request on May 6, 2024. In
a May 14, 2024 response the county noted that they had noted high flow values and after review found that
their scale was off. After installing a new flow meter in mid-2021 their flows dropped significantly.
With this renewal, the Division has determined that WTPs will begin to have flow limits instituted. In general
the Division approach will be to take the maximum monthly flow for the most recent three years of flow data
and add 15% to this value. For the Sparks Road WTP the permit writer has taken into account the significant
decline in flow values with the installation of the new flow meter in mid-2021. The flow limit has been based
on the maximum monthly average flow since that time which was 0.48 MGD. Adding a 15% buffer to this
results in a Flow Limit of 0.55 MGD as a monthly average limit.
Whole Effluent Toxicity (WETS - The facility had four failed WET tests over the permit cycle including 2 failures
in the last six. The facility has passed the most recent four tests. This is a conventional WTP and by WTP

guidance and discussions with AT staff, these facilities should have pass/fail limit requirements. This change
has been made in the permit.
TSS and Turbidity - TSS and turbidity values have been well within acceptable ranges with the exception of one
TSS exceedance.
Nutrients (TN and TP) - These values have not shown any potential issues.
TRC - No potential issues shown with TRC values.
Reasonable Potential Analysis (RPA) - An RPA analysis was performed for three parameters - Copper, Zinc and
Fluoride. In the 2019 permit renewal the RPA analysis showed reasonable potential for Cu and Zn and permit
limits were added to the permit with a five year compliance schedule. It appears that the 2019 RPA may have
had some issues with the spreadsheet analysis as the Zn limits were set at 35 µg/L which seems to be very low.
It looks like some pieces of the spreadsheet were adjusted inaccurately which led to the use of dissolved
numbers that did not apply the translator to convert the value to total Zn.
For this RPA analysis data from the last three years (March 2021 - Feb 2024) was utilized including a flow value
of 1.38 MGD, the maximum monthly flow in this three-year period [Even though this value was high based on
inaccurate flow readings this value was used in the RPA to assess the most conservative results. Running with
the lower average of 0.55 used for the limit produced the same final results]. The three years of data also is
within the recorded data period where flow at the facility decreased significantly. So, this data set seems to best
represent the ongoing conditions at the facility. The receiving stream is a zero -flow stream, so all stream flow
values were set at 0.
Data for a few reported values for Cu and Zn were adjusted based on the county's response to an add -info
request about potentially inaccurate values. These changes were conversion of some values from mg/L to µg/L
and inclusion a few less than values. One low zinc value from September 5, 2023 was judged as questionable by
the permit writer and excluded from the RPA analysis as an outlier.
Cu. Zn and Fluoride: - The results of the RPA continue to show that Cu has reasonable potential with 19 of 35
values above the allowable level. Fluoride and Zn did not show reasonable potential to impact the receiving
stream. Monitoring for fluoride will continue to be included in the permit since fluoride is added to the treated
water. Zn will also be maintained in the permit for this permit cycle, but the permit limit will be removed since
there were no reported values above the allowable level and the maximum projected Zn level was well below
the allowable level. Monitoring will be maintained for Zinc, but the monitoring frequency will be reduced to
quarterly.
It is possible that adjustments in procedures and chemical use by the facility since the last permit have reduced
the potential risk for Zn. If the current numbers continue in the next permit cycle, then consideration can be
given to removing Zn monitoring from the permit. Ina May 8, 2024 response to an additional information
request the county verified that they had changed their chemical additions to utilize orthophosphate without
zinc. However, it appears that this has caused some other reactions in the plant that may lead to them
converting back to the use on zinc -orthophosphate.
NCG59 GENERAL PERMIT EVALUATION
As part of the permit application Davie County requested review of this facility for coverage under the
NCG59000 general permit. This facility is a conventional WTP which can be considered for coverage under the
general permit. The Sparks Road WTP continues to show reasonable potential for Cu to impact instream water
quality and requires permit limits for this parameter. In addition, the Sparks Road WTP has had four (4)
failures of WET tests over the permit cycle including two (2) of the last six (6) tests and three (3) of the last
twelve (12). The need for a permit limit for Cu and the past results for WET requirements exclude this facility
for coverage under the general permit at this time.

• July 10, 2002 - Permit Modification - This modification was issued to correct the settleable solids unit of
measure to ml/L.
• July 14, 2004 - Permit Renewal - This renewal added a permit limit of 17µg/L for TRC to be effective
February 1, 2006. The application notes the facility beginning to use ferric sulfate as a coagulant rather
than alum polymer.
• November 8, 2005 - Authorization to Construct (ATC) - The ATC is for the construction of a tablet
dichlorination system under the fast -track ATC process.
• November 13, 2009 - Permit Renewal - This permit renewal incorporates a number of changes based on
the 2009 WTP Guidance, including the addition of a number of monitoring parameters. Settleable solids
monitoring is removed as is the limit for turbidity. The permit also requires continuous flow monitoring to
be effective March 1, 2011. Staff with PWS had expressed some concerns with some of the monitoring

June 27, 2014 - Permit Renewal - This renewal changed the monitoring frequency for a few parameters
from 2/month to weekly based on the discharge values. Monitoring parameters remained the same with
the exception of the removal of aluminum (based on RPA results and changes in chemical usage). The
permit added a Reopener Clause due to WET testing failures. This permit also added the Electronic
Reporting Requirements to be effective April 1, 2015. The permit updated the components description and
chemical usage. RPA analysis showed RPA requiring monitoring for Cu, Fe and Zn. As in previous
applications, this one notes lagoons as designed to handle sludge for up to 10 years.
November 6, 2019 - Permit Renewal - This renewal added Cu and Zn limits based on the RPA analysis (see
discussion above about Zn). The permit included a compliance schedule for the limits to be effective
December 1, 2024. The schedule also included required submittal of a Corrective Action Plan. The permit
removed monitoring for iron and adjusted units of measure to assure submittal of metals as µg/L. The
permit added hardness monitoring for evaluation of metal concentrations and removed the reopener

November 25, 2020 - Corrective Action Plan - The plan was submitted in response to the facility working
toward compliance with permit limits for Cu and Zn. This plan discusses actions the Davie County Utilities
Department is taking working with a consultant to address alternatives to address compliance. The plan
includes looking at monitoring in additional locations, looking at optimizing performance of sedimentation
procedures and optimizing chemical usage including change to an orthophosphate product that doesn't
have zinc.
PROPOSED PERMIT CHANGES:
• Updated eDMR requirements to be consistent with final EPA rule.
• Added regulatory citations throughout permit as needed.

• Language on the Supplement to Permit Cover Sheet has been updated. This includes slight modification of
the treatment system components including the maximum monthly average flow. Based on Division
adjustment to the approach for WTPs, with this renewal the facility has been given a monthly average flow
limit of 0.55 MGD.
• Monitoring requirements have been adjusted to require the submission of all constituent parameters used
in the calculation of the TN value. TKN and NO3-N + NO2-N have been added to the Effluent Table and
must be reported separately in eDMR.
• Based on the results of the RPA analysis the limits for Zn have been removed and monitoring frequency
has been reduced to quarterly.
• In accordance with WTP Guidance the WET conditions for Conventional WTP should be pass/fail limited
requirements. The language in the Effluent Table and Section A.(2) for WET requirements has been
updated accordingly.
• The permit map and descriptive information on the permit map have been updated.
• The expiration date has been changed to August 31, 2029, to allow for a nearly full five-year permit term.
• Facility coordinates corrected in BIMS.

Just for my clarity, can you explain why a flow limit was just added and what is the basis for "adding a 15% buffer"
to determine this flow limit? Is there a concern about receiving water quality if all permit limits are met, but the
monthly average flow exceeds 0.55 MGD?

Good Morning Bradley, we have gone over the list of data questions you provided and did indeed find a
few discrepancies. I have utilized your spreadsheet to show which were reported incorrectly (4 dates
highlighted in red) , what the issue was as well as what the certified lab results showed for all. We can
scan any or all certified lab results in question and send them to you if you wish. Main issue is, on
occasion the lab results reflect mg/l and not ug/l as requested. The majority of the time results are in ug/l
as noted on most of the dates you sent but again on occasion they have to be converted and reported in
ug/l for eDMR's. There were a 3 that were not converted to ug/l and 1 converted incorrectly. Three of the
dates the "<" was omitted but correct units were reported on lab results sheet as well as on eDMR's.
I hope this helps and again we have all certified lab results on file for well over the past 10 years and can
easily scan an send to you.

Good morning Bradley, I have added our ORC for the NPDES Aaron Burris to this email chain. I will be
sitting down with Aaron to go over the Zinc and Copper data inquires and will respond with our findings.
In regards to flow, I contested the flow with our scada team as prior flows seemed extremely high for our
facility and did not add up with Backwashing and waste. It was found that the scale was way off, we
installed anew flow meter and made note of it on our Local Water Supply plan and I believe the eDMR as
well. So from 2021 on the flow is very accurate and that is why there was a significant drop in flow
totals.
In regards to chemicals used the only change that was made was going from the Zinc Orthophosphate to
a straight Orthophosphate for corrosion control. In the process we have found that there has been a
reaction in parts of our system with concrete lined ductile iron pipe that has resulted in pH
increases. We are wanting to go back to the Zinc orthophosphate for increased corrosion control for lead
and copper purposes.
We have not added any new chemicals in our facility or processes for Copper and Zinc.
Again I will look over the sheet sent in regards to Zinc and Copper results in question and will get back
with you as soon as possible.

While going through the information for your permit renewal we have a few questions while reviewing your
submitted discharge data. Could you please respond to these issues as soon as you can.
• Zinc and Copper data. — I have attached a spreadsheet with submitted results from a few days over the last
fewyears. Some of the data appears to have fairly low reported numbers. These numbers may be
accurate, but we want to make sure since they are actually below the Practical Quantitation Limits (PQLs)
for these parameters [Cu — 2 pg/L and Zn —10 pg/L]. Possibly some of the values are reported in the wrong
units. Values should be reported in pg/L.
• Flow— in reviewing the reported flow data it appears that the discharge from your facility decreased
significantly in the mid- 2021 timeframe through the current reporting. Where there some changes in your
system or operation that may have resulted in this reduction?
• In a Corrective Action Plan submittal in November of 2020 there is discussion about changing your
chemical usage to address possible zinc issues by moving to an orthophosphate that does not include
zinc. Could you please confirm if this change has been made. If so, we will adjust the chemical
components noted in the permit.
• If there are other changes that may have been incorporated in your treatment process or chemicals used
to address issues with copper and zinc limits, please summarize those for us as well.
Once we have received these responses, we can move forward with completing your draft renewal.

If a permit has TN monitoring, I would add TKN and Nitrite + Nitrate Nitrogen. They are the components that
make up TN and Mike/Nick mentioned that having the data for each of the components is helpful for the
basin planning branch. So I would think regardless of the receiving stream, if the permit is going to have TN,
it should also have those two. It will be talked about in the next staff meeting.
Ahh, I see now. If you were to only use data from the past 3 years when the flow was significantly flow, then
would update the flow number in the RPA as well. The high flow (1.384) as the Qw in the RPA wouldn't be
the best representation of flow over the three years (july 2021 to present).
Hopefully that makes sense? If not, call me and we can work through it. Maybe I'm still misunderstanding.

Thanks for the comments on this one. You would think at some point I'd get better at this. I'll make changes
where you pointed things out. I did have questions for you on a few items.
On the TKN and Nitrate + Nitrite — Should we include that on all permits with TN or just he ones that
discharge to NSW waters. Seems like we may only use that additional data at times in the NSW basins so I
was thinking if non -NSW then we could just leave as requiring TN submittal?
For my RPA analysis I noted in the Fact Sheet that the DMR data showed significant reduction in flow
starting in Mid-2021 (went from average flow of 1.2 MGD to average of 0.304MGD. Since there was such a
change in flow I decided to limit all the data used in the RPA to the last three years since it seemed more
representative of the current discharge situation. That is why I didn't use the last 4.5 years. Do you think
should still go back and use the older data when the flow was higher as well?
have a request into them for updates on the reported numbers for Cu and Zn that seemed off and have
also asked for more info on the significant drop in flow.

Sorry I didn't get this back to you until now! I started reviewing it on Tues, but never got a chance to finish until now
because I had to work on the NCG59 permit. I'm just going to say it's Mike and John's faults haha. My comments
are pretty much all minor corrects. The only major issue I found was with the RPA. It looks like it will need to be re-
run, which might affect the Copper, Fluoride & Zinc limits.
Permit:
• Missing the end ] for sodium hydroxide in the chemical list
• Facility Grade is missing from A. (1)
• TRC footnote could stand to be updated. The language looks different than the template language
• Add TKN and NO2-N + NO3-N monitoring to eff table
• BIMS still shows previous coordinates for facility location
RPA
• Stream Class is missing, although the WS box was still checked.
• Data from the past 4.5 years should be used in the RPA, so the missing data will need to be added. Only
Flow is the past 3 years.
• The Fluoride data in the RPA is still in mg/L. The values needs to be converted to ug/L for the RPA.
• For the Copper data point that shows an Error, it's likely that they reported it in incorrect units and it should
be 7 ug/L. I would either email the permittee and confirm the units, or remove it from the data set.
• A few of the Zinc values look incorrect. 0.24 on 9/5/2023 is well below the detection value. Same with 0.01
on 11/1/2022. Since there are multiple values that look incorrect, I would ask the permittee for those lab
reports to double-check them. I would also add a reminder of the Zinc PQL in the cover letter, as they were
previously only reporting down to 10 ug/L as recent as June 2023.

Question on Potential General Permit Coverage:
In the cover letter for your application you requested that your facility be considered for coverage under general
permit NCG590000. Your facility currently has requirements for permit limits for Total Zinc and Total Copper. In
review of your submitted DMR data it appears that these parameters will continue to be limited for your
facility. The requirement for these limits excludes your facility from being covered under the general permit. We
will continue to process your permit as an individual permit at this time.
Permit Contact Information Update:
Attached to this email is a document that provides you with the contact information for your facility that we
currently have in our permitting database. Could you please review this information and let us know of any
changes/deletions/additions that might be needed in this information. You can mark up the document and return
it through email if that is the easiest process for you.
