downstream) area .

PART IV - EVALUATION AND RECOMMENDATIONS
The applicant , Dewitt Moretz , has applied to renew the permit
which allows the discharge of treated domestic wastewater into a
cove of Lake Hickory . The applicant was issued a permit effective
April 1 , 1988 which required monitoring to be completed quarterly .
As of this report date no monitoring has been received by this
Office.
Due to the treatment units being underground an on-site
inspection can not reveal treatment unit effectiveness , especially
if no monitoring has been completed. However, the chlorination
unit contained chlorine tablets .
It appears that the facility had an active status in the
State ' s tracking system but was classified as special and this
Office had yet to conduct a compliance inspection. This Office
will contact the appropriate persons in Raleigh to change the
tracking to active and the proper notifications (NOV's) can begin
being sent upon failure to submit self-monitoring. This Office is
going to send a NOV to the permittee .
It is recommended that the permit be renewed.
