I wish to not renew the boiler blowdown discharge permit.

The boiler has not been operated since Hooker Furniture
last used it in 2005. I purchased the facility in 2006 from
Hooker Furniture. The property basically remains vacant
at this time.

Larry — Thanks for talking with me this afternoon. It is my understanding that you wish to keep the above
general permit current should you need to discharge boiler blowdown from the Thrower Road facility in the
future. As I explained, the permit expires on July 31, 2015. However, before the Division can renew the
permit, the delinquent annual fees for 2013, 2014 and 2015 will need to be paid. The invoices are attached
and should to be processed immediately. Upon payment, the new permit will be sent to the 100 Ashe Place
address. Let me know if you have any questions. Thanks, Mike.

On December 5, 2012, Mike Mickey of this office met with you to perform a General Wastewater
Permit Inspection at the Pleasant Garden Enterprises facility at 1212 Thrower Road. Currently, the boilers
are not operated. Hooker Furniture previously fired the boilers with sawdust prior to closing in 2005. At
present you are operating a printing operation in one area of the facility that is heated using a space heater.
It is our understanding that there are; no plans to operate the old boilers. If that is the case, the
present permit could be voided. Attached is the necessary rescission form. Please complete and mail to
the address listed in Raleigh. Once rescinded, the annual permit fees will cease.
If at some future time it becomes necessary to discharge non -contact cooling water, cooling tower or
boiler blowdown wastewater, you can simply reapply for coverage under the general permit.

Comment: The former Hooker Furniture facility closed in 2005. Previously, they had a
discharge from boilers fired by sawdust. The discharge ceased at closing. The building
was bought by Pleasant Garden Enterprises, LLC (Larry Booth). Currently, Larry is
operating a very small printing operation in one room of the building on the south side of
Thrower Road that is heated using a space heater. (Note: he also owns the building on
the north side of Thrower Road, but has nothing to do with the lumber drying operation
that is adjacent to the north building). Since no discharge is planned in the near future,
a rescission form is to be sent.

Certificate of Coverage (CoC) NCG500612 to discharge under NCG500000. This CoC is issued pursuant to the
requirements of North Carolina General Statue 143-215.1 and the Memorandum of Agreement between North
Carolina and the US Environmental Protection agency dated May 9, 1994 [or as subsequently amended].
If any parts, measurement frequencies or sampling requirements contained in this General Permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual permit
application. Unless such demand is made, the certificate of coverage shall be final and binding.
Please take notice that this Certificate of Coverage is not transferable except after notice to the
Division. The Division may require modification or revocation and reissuance of the certificate of coverage.
Contact the Winston-Salem Regional Office prior to any sale or transfer of the permitted facility.
Regional Office staff will assist you in documenting the transfer of this CoC.
This permit does not affect the legal requirements to obtain other permits which may be required by

BILL OF SALE
This BILL OF SALE (the `Bill of Sale") is executed and delivered pursuant to
Section 0(c)(ii) of the Purchase Contract, dated as of November 8, 2005 (as heretofore amended
and supplemented, the "Purchase Contract"), originally between Hooker Furniture Corporation, a
Virginia corporation ("Seller" ), and Larry D Booth, an individual doing business as Booth
Enginecring Co., a sole proprietorship, and subsequently assigned to and assumed by Pleasant
Garden Enterprises, 11,C, a North Carolina limited liability company ("Buyer"), under the
Assignment and Assumption of Purchase Contract and Fourth Amendment to Purchase Contract,
dated as of May 17, 2006.

respective meanings given to them in the Purchase Contract
2. Sale and Transfer of Personal Property. For the consideration set forth in the
Purchase Contract and other good and valuable consideration, the receipt, adequacy and
sufficiency of which are hereby acknowledged, Seller hereby sells, transfers, conveys, grants and
delivers to Buyer, effective as of 12:01 a.m. EDT on May 17, 2006, all of Seller's right, title
and interest in and to all of the personal property (the "Personal Property") described on
Schedule A hereto.
3, Further Actions. Seller covenants and agrees to warrant and defend the sale,
transfer, conveyance, grant and delivery of the Personal Property hereby made against all
persons whomsoevet claiming by, through or under Seller, but not otherwise, to take all steps
reasonably necessary to establish the record of Buyer's title to the Personal Property and, at the
request of Buyer, to execute and deliver futther instruments of transfer and assignment and take
such other action as Buyer may reasonably request to mote effectively transfer and assign to and
vest in Buyer the Personal Property, all at the sole cost and expense of Seller.

In accordance with your application for a discharge permit received on March 27, 2001
we are forwarding herewith the subject certificate of coverage to discharge under the subject state
— NPDES general permit. This permit is issued pursuant to the requirements of North Carolina
General Statute 143-215 .1 and the Memorandum of Agreement between North Carolina and the
US Environmental Protection agency dated May 9, 1994 (or as subsequently amended).
This general wastewater permit for boiler blowdown replaces your previous individual
wastewater permit, NC0001171, which is hereby rescinded since your wastewater is now
discharging to the City of Greensboro POTW.
If any parts, measurement frequencies or sampling requirements contained in this permit
are unacceptable to you, you have the right to request an individual permit by submitting an
individual permit application. Unless such demand is made, this certificate of coverage shall be
final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to
the Division of Water Quality. The Division of Water Quality may require modification or
revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be
required by the Division of Water Quality or permits required by the Division of Land
Resources, Coastal Area Management Act or any other Federal or Local governmental permit
that may be required.
