pursuant to the requirements of North Carolina General Statue 143-215.1 and the Memorandum of
Agreement between North Carolina and the US Environmental Protection agency dated October 15, 2007 [or
as subsequently amended].
If any parts, measurement frequencies or sampling requirements contained in this General Permit
are unacceptable to you,you have the right to request an individual permit by submitting an individual
permit application. Unless such demand is made, the certificate of coverage shall be final and binding.
Please take notice that this Certificate of Coverage is not transferable except after notice to the
Division. The Division may require modification or revocation and reissuance of the certificate of coverage.
Contact the Winston-Salem Regional Office prior to any sae or transfer of the permitted facility.
Regional Office staff will assist you in documenting the transfer of this CoC.
This permit does not affect the legal requirements to obtain other permits which may be required by
the Division of Water Quality or permits required by the Division of Land Resources, Coastal Area
Management Act or any other Federal or Local governmental permit that may be required.

21 or regulations of the Environmental Management Commission implementing that Article, or who
falsifies, tampers with or knowingly renders inaccurate any recording or monitoring device or
method required to be operated or maintained under Article 21 or regulations of the Environmental
Management Commission implementing that Article, shall be guilty of a misdemeanor punishable

Dear Ms. Hudson:
1. Ron Boone of the Winston-Salem Regional Office of the NC Division of Water Quality (DWQ)
conducted a compliance evaluation inspection (CEI) at the subject facility on 18 August 2009.
You were not present for the inspection. An inspection checklist is attached for your records
and the inspection findings are summarized below.
2. The facility is located at 228 New Heights Drive, Reidsville, Rockingham County, NC. The
facility discharges treated domestic wastewater to an unnamed tributary to Wolf Island Creek,
which is currently classified as a Class C water in the Roanoke River Basin.
3. Mr. Boone was not able to locate the septic tank, sand filter or discharge pipe for the system.
He was able to locate the chlorinator, but could not open the tubes for inspection. Please keep
in mind that failure to pump the tank at least every five years is not only a violation of the
general permit, but could also cause irreparable damage to downstream treatment units that
would be costly to replace, as well as cause unsanitary conditions at the residence for you and
your family. You should maintain records of all tank pumping along with all other records (i.e.
technical drawings/specifications) you have for the system so they're readily available during
future inspections by this office.
4. Also, please note that you should use the 2-5/8 inch or 3 inch diameter chlorine tablets that are
rated for use with wastewater. Using tablets rated for swimming pools is more expensive and
less efficient. Also, you should keep both chlorinator tubes stocked with tablets to a level
slightly above the normal water level in the chlorinator. This provides more chlorine contact
than using just a single tube. If the tablets you're using are not rated for use with wastewater

(i.e. rated for use with swimming pools), please use attachment #3 to find a supplier for tablets
that are rated for wastewater. This rating should be indicated on the container or the supplier
should be able to tell you whether they're or not.
5. According to our files, no analytical monitoring of the discharge has been accomplished to
date. Please take samples and have them analyzed as required in the permit (Attachment #4)
at your next possible sampling opportunity.
6. Please note that your permit for this facility is currently expired. The permit expired 31 July
2007. Furthermore, annual fees for the permit have not been paid for the years 2005-2009.
You are therefore discharging illegally. To renew the permit, you must fill out and submit
the attached renewal form (Attachment #2). Please follow all instructions on the form very
carefully. Upon receipt of the completed form, DWQ's Central Office (CO) will put the
permit/COC in your current name and send it to you.
7. Please be aware that the Division may assess civil penalties of up to $25,000 per day, per
violation for failure to comply with the requirements of NCG550000, as well as for discharging
without a valid permit. Please review the NCG550000 general permit in detail to familiarize
yourself with all requirements and to ensure you maintain future compliance. If this office, or
DWQ's CO, does not receive correspondence from you within 30 days of your receipt of this
letter, we will recommend to the Director that enforcement action be taken against you.

The Division is auditing its files regarding single-family residences [and similar discharges] covered under
General Permit NCG55O000. During our file review it was determined that:
> The Division has no record of your having a valid permit for the discharge of wastewater from your
residence. Your residence was previously covered by Certificate of Coverage (CoC) NCG550179; that
CoC expired on July 31, 2007.
> Annual fees for the years 2005-2009 have not been paid.
There are serious compliance issues and violations noted as follows.
Discharging Without a Permit
You are hereby advised that the discharge of wastewater without a permit from the Division constitutes a
violation of North Carolina General Statute (NCGS) § 143-215.1 [Control of sources of water pollution;
permits required], which states:
(a) Activities for Which Permits Required. — No person shall do any of the following things or carry
out any of the following activities unless that person has received a permit from the Commission and has
with all conditions setforth in thepermit:

(1) Make any outlets into the waters of the State.
(2) Construct or operate any sewer system, treatment works, or disposal system within the
State.
(6) Cause or permit any waste, directly or indirectly, to be discharged to or in any manner
intermixed with the waters of the State in violation of the water quality standards
applicable to the assigned classifications or in violation of any effluent standards or
limitations established for any point source, unless allowed as a condition of any permit,
special order or other appropriate instrument issued or entered into by the Commission
under the provisions of this Article.
The enforcement options available to the Division for violations of§ 143-215.1 are found at NCGS 143-215.6
A, B, and C, and include civil penalties, criminal penalties, and/or injunctive relief. The civil penalties
for violations such as these may be as high as $25,000.00 per day per violation.

Weaver at the address indicated on the Form within 10 days of receipt of this notice.
• With the renewal form you should submit payment for the unpaid annual fees,which total$270.00.
Make checks payable to NC DENR;include the invoice number[s] on your check[s].
The Division is considering the pursuit of punitive sanctions against you. If we do not receive
the items listed above by August 19. 2009.a civil penalty will likely be assessed.
