The Division has reviewed your permit rescission request received on
April 30, 2015. The Division has no objection to your request. Therefore,
NPDES permits NC0003051 and NCO086801 are rescinded, effective immediately.
If in the future your municipality wishes to discharge wastewater to
the State' s surface waters, it must first apply for and receive a new
NPDES permit.

The Town of Mount Olive holds NPDES Permits for the discharge of backwash from two water treatment
plants:
- NPDES Permit NCO086801 Gordon Street WTP
311 Gordon Street, Mt. Olive, NC
- NPDES Permit NC0003051 Mt Olive Water Treatment Plant #3
179 Northeast Church Street, Mt. Olive, NC
The backwash discharge lines for these facilities are also connected to the Town's sanitary sewer
collection system. For the recent past, the Town has chosen to discharge to the collection system and not
to have a direct discharge from these facilities.
The Town is requesting that both of the subject NPDES permits be rescinded. The backwash discharge
has had no effect on the wastewater treatment plant and the Town will continue to discharge to the
collection system.

This is just a reminder to the Town...
Last month the Division spoke with Dwight Lancaster and we were told that the missing DMRs (see below) were going to
be sent in in mid -July.
To date, we have not received them.
In that same conversation we discussed a potential settlement on the enforcement case LR-2015-0003. The Division
needs the missing DMR's to complete our files. Since there is no discharge at these plants the Town may submit the
missing reports (2 copies) at any time. Once the Division receives them, you will be contacted to negotiate settlement of
the penalty.

Thank you for the information provided below. I have suspended the monitoring requirement effective 7/1/15 for both
permits.
Please be sure to provide all DMRs through June 2017.

Once you are ready to rescind the permits, please contact the Washington Regional Office to determine whether a
closure inspection is required. Once you confirm that, the permittee needs to submit a written request to rescind the
permits.

The Town of Mount Olive has two water treatment plants with NPDES permits:
Gordon Street WTP - #NC0086801 and
Mt. Olive WTP #3 - #NC0003051.
Although the WTPs have discharge permits, the backwash from these faculties have been directed to the Town's WWTP
for the past few years. There has been no direct discharge from either of these WTPs and it is not anticipated that the
WTPs will go back to direct discharge. The Town will sent a request in the near future requesting that both of the
permits be rescinded. In the meantime, we request that the monitoring requirement for both of the NPDES permits be
suspended.

Dear Mr. Brown:
The Division of Water Quality (the Division or DWQ) hereby issues the attached NPDES permit for the
subject facility. We issue this permit pursuant to the requirements of North Carolina General Statute 143-
215.1 and the Memorandum of Agreement between North Carolina and the U.S. Environmental
Protection Agency dated October 15, 2007, or as subsequently amended.
Changes to Your Previous Permit. Based on your renewal application received August 2, 2012, we have
revised your permit in accord with the Division's current permitting guidance (October 2009) for potable -
water treatment plants (WTPs). We understand that you have made no significant alterations to your
wastewater treatment facilities since last renewal (2006). We have updated your facility description and
map, and added parameter codes to the effluent monitoring pages for your convenience [see Supplement
to Cover Sheet and permit § A. (1.)].
If any parts, measurement frequencies, or sampling requirements contained in this permit are
unacceptable, you have the right to an adjudicatory hearing, upon written request submitted within thirty
(30) days after receiving this letter. Your request must take the form of a written petition conforming to
Chapter 150B of the North Carolina General Statutes, and you must file it with the office of
Administrative Hearings, 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such a
demand is made, this permit shall be final and binding.
This permit is not transferable except after notifying the Division of Water Quality. The Division may
require permit modification, or revocation and re -issuance. Please note that this permit does not affect
your legal obligation to obtain other permits required by the Division of Water Quality, the Division of
Land Resources, the Coastal Area Management Act, or other federal or local governments.

4. Total Residual Chlorine (TRC) - The Division shall consider all effluent TRC values reported below 50µg/L to be
compliant with this permit. However, the Permittee shall continue to record and submit all values reported by a North
Carolina -Certified laboratory test method (including field certified), even if these values fall below 50µg/L.
5. To be monitored only if the Permittee backwashes filter -medium with water containing aluminum sulfate.
6. To be monitored only if the Permittee adds zinc orthophosphate as a corrosive inhibitor prior to backwashing.
7. To be monitored only if the Permittee backwashes with finish water containing fluoride.
Conditions:
1. All samples shall be collected during the backwash cycle.
2. The Permittee shall discharge no floating solids or foam.
3. The Division may, upon written notification to the Permittee, re -open this permit in order to incorporate or modify
effluent limits, monitoring and reporting requirements, and other permit conditions when it deems such action is
necessary to implement TMDL(s) approved by the USEPA.

Permittee confirmed green -sand filters (31Jan2012).
Correspondence with the Permittee — the Application for renewal received August 02, 2011 indicates no
changes to wastewater treatment facilities since last renewal. Joe called Deem Blanton (WTP Superintendent) to
discuss; he said this facility uses four (4) "regular iron -removal" sand filters (smaller - 6k gpd) and four (4)
greensand filters (larger 9-10 gpd). This plant is also connected to the city's WWTP via sewer providing the
option of "no discharge" [recently, since July 2011, DMRs reported as "no flow"]. The Permittee wishes to
retain these discharge options. The Division has no objections.
Proposed Renewal - Changes from the Draft Permit — the Division will revised the permit and update the
effluent page to reflect the Water Treatment Plant (WTP) Permitting procedures 2009.Only Total Iron data are
available for Reasonable Potential Analyses (RPA) — no RP. No other parameters of concern.
Green -sand filter systems do not require Whole Effluent Toxicity (WET) testing.

The Town of Mount Olive is requesting renewal of NPDES permit number NCO086801 for the backwash flow
from the Gordon Street WTP. The plant treats groundwater which is filtered with a combination of sand and
greensand filters. The greensand filters are not backwashed with a sodium solution.
The finished water is being fluorinated at this time, but the WTP is considering discontinuing the addition of
fluoride. We are requesting that permit be written so that fluoride monitoring/limits will be applicable only if
the town is adding fluoride. The current permit has a weekly monitoring requirement for iron and settleable
solids. The results for the settleable solids tests are consistently measured as a "Trace". The Town is
requesting that settleable solids be removed from the permit. The Town is also requesting a change in the
measurement frequency for iron from weekly to quarterly.

Two wells pump raw groundwater to the Gordon Street Water Treatment Plant. At the
treatment plant, chlorine and flouride are added prior to filtration. The water is filtered
through eight (8) filters, of which four are greensand filters and four are conventional
sand filters. The filtered water is pH adjusted with caustic. Corrosion inhibitors are also
added after filtration.
8. Describe the wastewater and the treatment processes) for wastewater generated by the
facility:
Filter backwash water flows to a backwash holding tank. Declorinating chemical is added
to the backwash holding tank prior to each backwash cycle. The backwash is allowed to
settle, the clear supernatant is decanted to the receiving stream. The accumulated solids
in the backwash holding tank are discharged to the sanitary collection system.

The Town of Mount Olive operates the Gordon Street Water Treatment Plant which uses
groundwater as the raw water source. The raw groundwater is filtered through a combination of
conventional and greensand filters. During the backwash cycle, any accumulated solids are
discharged to the backwash holding tank. This tank allows the solids to settle and thicken in the
bottom of the holding tank. The sludge is discharged to the sanitary collection system as needed
for ultimate disposal at the Town's wastewater treatment plant.

The Division received your request for permit modification on December 9, 2010. Division staff
reviewed the request and the subject permit.
Unfortunately, at this time we cannot modify the permit. The Division's October 2009 Water
Treatment Plant (WTP) strategy has been applied to WTP's statewide as they have come up for renewal.
This permit has never had the strategy applied; therefore, it cannot be applied selectively (in this case to
settleable solids and iron). Your permit is due to expire January 31, 2012 and the strategy for
conventional water treatment plants will be applied to the next permit in its entirety (see Table 3.1).
For your reference, I have included a copy of the pages in our WTP Strategy that apply to
conventional water treatment plants. Deems Blanton was contacted on December 17, 2010 to confirm
that this WTP is a conventional plant as a previous renewal application stated it was a green sand filter
system. Mr. Blanton confirmed it is a conventional system and mentioned that the green sand was
changed out approximately 4 years ago.

We back wash our potable water filters daily with app..023 MG of potable water. That
water is sent to a holding tank, where we let it settle for approximately 23 hours. We
then turn the clear supernate off the top to a near by drainage ditch that has no flow at
the head of it. That ditch eventually drains into the North East Cape Fear River. The
settled iron sludge on the bottom of the tank is pumped once per month to the Town of
Mount Olive Waste Treatment Plant at app..006 MG.

Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the U.S. Environmental Protection Agency dated May 9, 1994 (or as subsequently amended).
This final permit includes two major changes from the draft permit sent to you on November 14, 2001.
Both of these changes were made at the request of the WTP operator:
❖ Turbidity monitoring has been replaced with a turbidity limit of 50 NTU at the effluent.
❖ Monitoring for aluminum has been deleted, as your facility does not use alum flocculation.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to
you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of
this letter. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina
General Statutes, and filed with the Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North
Carolina 27699-6714). Unless such demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may require
modification or revocation and reissuance of the permit. This permit does not affect the legal requirements to obtain

The Town of Mount Olive wishes to renew permit # N.C. 0086801 for its' Water
Treatment Plant # 1 located at 311 Gordon Street. There have been no changes in the way the
plant operates since the permit was issued.
The Town backwashes its' potable water iron removal filters at a rate of approximately
twenty five thousand (25,000) gallons per day into a holding basin. We then let it settle for
twenty four (24) hours and then turn the clear supernate into a nearby field ditch that eventually
reaches a tributary of Cow Pasture Branch which leads to the North East Cape Fear River. The
heavy iron sludge that settles on the bottom of the basin is pumped to the Town's Waste Water
Treatment Plant at approximately five thousand (5000) gallons per month. The above procedures
were approved by the State.

Notes for Town of Mount Olive Water Treatment Plant
General Information
The Town of Mount Olive has requested that an NPDES permit allowing the facility to
discharge filter backwash water. Previously, this facility discharged the filter backwash
water to the Town of Mount Olive's WWTP, however the WWTP is currently under an
SOC because of excessive flows. Removing the water treatment plant contribution will
provide interim relief to the WWTP.
The facility is proposing to discharge 0.02 MGD of filter backwash water into an
unnamed tributary to the Northeast Cape Fear River, which is a class C Swamp water, in
the Cape Fear River Basin. The Town of Mount Olive is located in the headwaters of the
Northeast Cape Fear.

General Comments
This permit will be developed using the water treatment plant permitting policy. In the
future if the WWTP should correct the problems, the permittee should be encouraged to
again begin discharging to the sanitary sewer.
Per Charles Weaver, this request requires a new permit application. Additionally,
Charles indicated that he and Dave Goodrich have concluded that an engineering
alternatives analysis is not required. Based on this information it is assumed that the only
option for the facility is discharge and permit development will proceed accordingly.
2/26/00 — contacted the Town of Mount Olive about the situation with the chlorides in
Barlow Branch and the Northeast Cape Fear River. This is seen as a permanent measure
being employed by the facility. Chad Winem informed the Division that the water
treatment plant does not practice brine recharge as part of the water treatment process and
chloride levels in the wastewater have been below detection limits.
2/26/00 — using a 1:20,000 USGS topographic map the watershed was delineated and
determined to be 0.0133 mil. Referring the Low Flow Characteristics of Streams in
North Carolina for hydrologic area 2 (HA2), drainage areas less than 2.0 mi2 would be
expected to have a 7Q10 and 30Q2 equal to zero.

SUMMARY
The Town of Mount Olive has requested that an NPDES permit allowing the facility to discharge
filter backwash water. Previously, this facility discharged the filter backwash water to the

Barlow Branch and 3.3 miles of the Northeast Cape Fear River from the source to SR1937 are
rated as not supporting their designated uses due to elevated level chloride levels from the
Mount Olive Pickle Company. The facility is proposing to discharge 0.02 MGD of filter
backwash water into an unnamed tributary to the Northeast Cape Fear River, which is a class
C Swamp water, in the Cape Fear River Basin. The receiving stream, UT to the Northeast Cape
Fear River, discharges into the Northeast Cape Fear River in a portion of the river, which has
elevated chloride levels. Since the water treatment plants does not utilize brine recharge as a
unit operation chloride levels in the effluent are minimal and the discharge should have a
positive impact on the chloride levels by increasing dilution.

This discharge results from the backwash of filters at a Municipal water treatment plant.
The discharge is able to meet proposed permit limits based on data submitted. Currently this
backwash water is being discharged to the Mt. Olive WWTP. However the WWTP is
hydraulically over loaded and has man hole overflows during heavy rainfall events. It is felt
that it is better to discharge this relatively clean water directly to the receiving stream and
reduce the hydraulic load on the WWTP. It is recommended that the permit be reissued.

Please find enclosed three (3) copies of the Application for Permit to Discharge — Short Form A
along with a map showing the location of the proposed discharge and a $715.00 check for the
application fee. This request will allow the Town of Mount Olive to discharge filter backwash
water to a wet -weather ditch that flows to the Northeast Cape Fear River. The backwash water
from this plant was once permitted to be discharged to the ditch under NPDES permit No. NC
003051, discharge No. 1. The discharge was moved to the sewer for convenience some time ago.
The sewer system is now under and SOC for excessive flows, and the Town needs to remove this
filter backwash from the water system.
The Town owns and operates a water distribution system that utilizes wells as its source of water.
At the Gordon Street Water Treatment Plant, there exist four (4) iron removal pressure filters that
presently discharge 20,000 gallons per day (gpd) to the sanitary sewer system as well as a 60,000-
gallon holding tank for the filter backwash water. Solids in the backwash water are allowed to
settle prior to being discharged into the sanitary sewer system.
Piping modifications to the holding basin are proposed so that supernate can be removed and the
solids can be collected in the bottom. Approximately 20,000 gallons of backwash water would be
discharged to the holding tank daily, allowed to settle and then discharged to the ditch. After one
(1) month, approximately 20,000 gallons of collected solids would be discharged to the sanitary
sewer system.

For one (1) month, the Town modified the piping to allow the solids to settle and the supernate to
be removed. The supernate was sampled and discharged into the sanitary sewer system.
Enclosed is a copy of the sampling data. The sampling data indicates the supernate would be able
to meet the existing NPDES limits for well No. 3. Well No. 3 discharges under permit NO. NC
003 05 1, discharge No. 2. Discharge No. 1 was originally for the same water treatment plant that
the application is currently being made.
