NPDES permit.

I am requesting that the Discharge Permit for Seagrove Elementary School (NC0040924) be rescinded.
We are no longer discharging effluent from the treatment system due to the installation of a pump
station. We are pumping effluent to Seagrove -Utah Metro Water District's new pump station located on
East Avenue, Seagrove NC.
Our engineer sent our certification letter to Raleigh some time ago when construction was complete,
but nothing has made it to your office, my apologies for that. Hopefully this letter will be satisfactory for
the file, but if you need any more information or if I can be of assistance please don't hesitate to let me
know.

Dear Mr. Chilton:
Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to
the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between
North Carolina and the U.S. Environmental Protection Agency dated October 15, 2007 (or as subsequently
amended).
This final permit includes the following changes from the draft permit sent to you on
July 6, 2011:
➢ Specific dates for the construction project to connect this facility to municipal sewer (and
terminate the discharge) have been added to A. (3), based upon the permit effective date.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30)
days following receipt of this letter. This request must be in the form of a written petition, conforming to
Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings
(6714 Mail Service Center; Raleigh, North Carolina 27699-6714). Unless such demand is made, this
decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may
require modification or revocation and reissuance of the permit. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or permits
required by the Division of Land Resources, the Coastal Area Management Act or any other Federal or Local
governmental permit that may be required. If you have any questions concerning this permit, please

All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby
revoked. As of this permit issuance, any previously issued permit bearing this number is no longer
effective. Therefore, the exclusive authority to operate and discharge from this facility arises under the

A. (2) NUTRIENT REOPENER CONDITION
Pursuant to N.C. General Statutes Section 143-215.1 and the implementing rules found in
the North Carolina Administrative Code at 15A NCAC 2H.0112 (b) (1) and 21-1.0114 (a) and
Part II sections B-12 and 13-13 of this permit, the Director may reopen this permit to require
supplemental nutrient monitoring of the discharge. The purpose of the additional
monitoring will be to support water quality modeling efforts within the Cape Fear River Basin
and shall be consistent with a monitoring plan developed jointly by the Division and affected
stakeholders. In addition, the results of water quality modeling may require that limits for
Total Nitrogen and Total Phosphorus be imposed in this permit upon renewal.

A. (3) PLAN FOR ELIMINATION OF DISCHARGE
Based upon the Permittee's stated intention to eliminate this discharge via
connection of the Seagrove Elementary School WWTP to a municipal sewer system,
the following target dates will be met:
Begin Construction: October 1, 2012.
Complete Construction: April 1, 2013.
Cease Discharge: January 1, 2014.
If adjustment of the dates is necessary due to events beyond the control of the
Permittee, a minor modification may be requested to re -open and modify this permit
condition.

Per our meeting back on 6/17/11 - instead of issuing a SOC we decided to place milestone dates in the next NPDES
permit for Seagrove Elementary. The school system is still trying to firm up the dates. In the interim, if you need to
move ahead, the suggested dates to include in the draft permit are:
Begin Construction —12 months after issuance
Complete Construction —18 months after issuance
Cease discharge — 2 years after issuance
Any need to include a date for submittal of a sewer extension application?

Charles —The existing permit for Seagrove Elementary School currently requires "Daily" effluent monitoring for
temperature. Can that be changed to "Weekly" in the new permit? This is a recirculating surface sandfilter with UV
disinfection. We recommended that the ORC request this change at renewal but he forgot. Would you need a written
request from the Randolph County BOE ? Thanks, Mike.

SLUDGE MANAGEMENT PLAN
SEAGROVE ELEMENTARY SCHOOL (NC0040924)
Our wastewater operators monitor the wastewater treatment daily and check
the scum blanket and sludge depth. When needed the sludge is pumped by a
local septic tank service; currently Vann Boys Septic Tank Service. The
sludge is then taken to the City of Asheboro's treatment plant for further
decomposition.

Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the U.S. Environmental Protection Agency dated May 9, 1994 (or as subsequently amended).
This final permit includes no major changes from the draft permit sent to you on July 5, 2006.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable
to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt
of this letter. This request must be in the form of a written petition, conforming to Chapter 150B of the North
Carolina General Statutes, and filed with the Office of Administrative Hearings (6714 Mail Service Center, Raleigh,
North Carolina 27699-6714). Unless such demand is made, this decision shall be final and binding.

SLUDGE MANAGEMENT PLAN
SEAGROVE ELEMENTARY SCHOOL (NC0040924)
Our wastewater operators monitor the wastewater treatment daily and check the scum
blanket and sludge depth. When needed the sludge is pumped by a local septic tank
service; currently Delk Septic Tank Service. The sludge is then taken to the City of
Asheboro's treatment plant for further decomposition.

An Authorization to Construct request, ATC fee ($150), and five sets of plans were received
on April 22, 1997 for the construction of a control building to house two UV disinfection units and
associated pumps, piping, and appurtenances for the above referenced batch recirculation project.
The facility was issued an Authorization to Construct for a UV system and control building
July 29, 1996 and was issued an Authorization to Construct for refurbishment of the sand filter
November 16, 1990. Another ATC is not required at this time. The facility may operate the
existing recirculating sand filter in a batch mode of operation as long as the permit limits are
consistently met.

A letter of request for an Authorization to Construct was received August 16, 1990 by the
Division and final plans and specifications for the subject project have been reviewed and found to
be satisfactory. Authorization is hereby granted for the construction of additions to the existing
wastewater treatment facility consisting of a 10,000 gallon (approximate volume) grease trap,
continued use of the existing septic tank, refurbishment of the existing sand filter and replacement
of dosing lines, a new 11,720 gallon (approximate volume) recirculation/dosing tank with duplex
pumps, audible and visual highwater alarms, and a ball float valve recirculation control with
continued discharge of treated wastewater into an unnamed tributary to Fork Creek.
This Authorization to Construct is issued in accordance with Part III paragraph B of
NPDES Permit No. NCOO40924 issued May 31, 1988, and shall be subject to revocation unless
the wastewater treatment facilities are constructed in accordance with the conditions and limitations
specified in Permit No. NCOO40924.
The sludge generated from these treatment facilities must be disposed of in accordance with
G.S. 143-215.1 and in a manner approved by the North Carolina Division of Environmental
Management.
In the event that the facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Permittee shall take immediate corrective action, including those as may be
required by this Division, such as the construction of additional or replacement wastewater
treatment or disposal facilities.

The Winston-Salem Regional Office, phone no. (919) 761-2351, shall be notified at least
forty-eight (48) hours in advance of operation of the installed facilities so that an in -place
inspection can be made. Such notification to the regional supervisor shall be made during the
normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State
Holidays.
Upon completion of construction and prior to operation of this permitted facility, a
certification must be received from a professional engineer certifying that the permitted facility has
been installed in accordance with the NPDES Permit, this Authorization to Construct and the
approved plans and specifications. Mail the Certification to the Permits and Engineering Unit,
P.O. Box 27687, Raleigh, NC 27611.
The Permittee shall employ a certified wastewater treatment plant operator to be in
responsible charge of the wastewater treatment facilities. The operator must hold a certificate of the
grade at least equivalent to the classification assigned to the wastewater treatment facilities by the
Certification Commission. Within thirty days after the wastewater treatment facilities are 50%
complete, the Permittee must submit a letter to the Certification Commission which designates the
operator in responsible charge.
A copy of the approved plans and specifications shall be maintained on file by the Permittee
for the life of the facility.
The sand media of the surface sand filter must comply with the Division's sand
specifications. The engineer's certification will be evidence that this certification has been met.
A leakage test shall be performed on the dosing tank to insure that any exfiltration occurs at
a rate which does not exceed twenty (20) gallons per twenty-four (24) hour per 1,000 gallons of
tank capacity. The engineer's certification will serve as proof of compliance with this condition.
Failure to abide by the requirements contained in this Authorization to Construct may
subject the Permittee to an enforcement action by the Division of Environmental Management in
accordance with North Carolina General Statute 143-215.6.
The issuance of this Authorization to Construct does not preclude the Permittee from
complying with any and all statutes, rules, regulations, or ordinances which may be imposed by
other government agencies (local, state, and federal) which have jurisdiction.

Dear Mr. Kerns:
The Division is beginning the cycle of permit renewals for the Cape Fear river basin. An examination of the Cape
Fear basin plan and the NPDES permit schedule has revealed that the existing expiration dates in Cape Fear NPDES
permits do not coincide with the revised permit expiration dates for the basin. This discrepancy could create unnecessary
delays in the processing of over 200 permit renewals. Accordingly, the Division is changing the permit expiration dates for
NPDES permits in the Cape Fear river basin.
This permit modification changes the expiration date of the subject permit in subbasin 30609 to September 30, 2001.
Please find enclosed the revised permit cover page. Insert the new cover page into your permit and discard the old page.
All other terms and conditions contained in the original permit remain unchanged and in full effect. This permit
modification is issued under the requirements of North Carolina General Statutes 143-215.1 and the Memorandum of
Agreement between North Carolina and the U. S. Environmental Protection Agency.
In accordance with the modification of the permit expiration date, the new due date for your renewal application is
April 3, 2001. Renewal application forms will be sent to you approximately 6-8 weeks in advance of the new due date or
you may download the forms from the NPDES web site at http://h2o.enr.state.nc.us/NPDES/documents.html. You
will need to download Short Form D. If you have already submitted a permit renewal form, there is no need to submit
any other forms to the Division; the application will be processed during the renewal period for your subbasin.
If any part of this permit modification are unacceptable to you, you have the right to an adjudicatory hearing upon
written request within thirty (30) days following receipt of this letter. This request must be a written petition conforming
to Chapter 150B of the North Carolina General Statutes, filed with the Office of Administrative Hearings (6714 Mail
Service Center, Raleigh, North Carolina 27699-6714). Unless such demand is made, this decision shall be final and binding.
