The Central Office should note that Mr. Enyarts address changed to 1401 Golf Course Road for 911
addressing purposes.
This system consists of a septic tanks, primary sand filter and discharge pipe. The facility was found to be in
good condition. Mr. Enyart informed us that he had recently had his septic tank cleaned. The discharge pipe
for the system could not be found. A copy of the orginal drawings of this facility have been attached , it is
requested that you attempt to locate your discharge pipe, mark it and keep it clear.
