Valves will be installed to allow pumping as is and/or directly to the flow equalization basins. Two variable speed pumps will be installed (one for each flow-equalization tank) to pump out of the basins to the existing flume that discharges to die distribution box. The current pre-acration basins are not in use and can be easily converted to flow equalization basins. The current filtrate flow from the gravity belt thickener (GBT) and belt filter press (BFP) operation is currendy pumped from the Drain Pump Station to die Preliminary Treatment Facility and then gravity flows to the distribution box and onto the primary clarifiers. This creates a highly concentrated plug flow. Tlie effluent from the GBT and BFP is sufficiently filtered to be pumped directly into the re-purposed pre-aerauon basins (flow-equalization basins) with sufficient storage volume to allow them to pump out of the flow-equalization basins to the distribution box using variable speed pumps to obtain more of a continuous feed. This flow equalization will improve treatment. The flow equalization basins are equipped with an overflow flume that flows to the distribution box. In the event that die flow-equalization basins become overfull, the effluent will simply discharge to the distribution box via the overflow flume. The attached spreadsheet details all of the sampling conducted with a map showing sampling points. It was determined that the event had already migrated down the ditch towards the junction with Toisnot Swamp. At 1100 a pump starting transferring water out of the ditch at site #2 at a rate of 250 gpm. This water was being discharged into the sanitary sewer system. Monitoring was conducted in the sewer system ensuring no permit limits were exceeded. At sample site #1, 3,000 gallons was removed to aid in remediation at the source. Two more pumps were added at site #2 at 1300, for a total of three pumps or approximately 750 gpm being withdrawn. Throughout the course of the day, sampling continued. There were two dead fish noted in the ditch and two ducks were noted swimming at the junction of the ditch and the stream. The pumps were secured at 2000 having pumped a total of 350,000 gallons from the ditch to the sanitary sewer system. I was informed of the incident at 0750 on May 14, 2014. It was said initially that ~3,000 gallons had been released. After review of the incident, 1821 gallons (4,826 dry lbs) was determined to have been released. Barry Parks-Director of Water Resources was informed at 0840 of the incident. Mike Hicks with NCDENR-PWS was notified at 0900. He contacted NCDENR-DWR forguidance. I spoke with Autumn Romanski with NCDENR-DWR at 0902 and filled her in on what was known. She stated to follow the MSDS for the chemical and keep her informed. The MSDS states to follow local and state guidelines. On May 13, 2014 at 0320 the operator at the Toisnot WTP stopped recirculating a sodium hydroxide (caustic soda) bulk storage tank. The operator failed to close one of the valves. The mistake was not noticed until the next night shift at 0030 on May 14, 2014. At this point 1821 gallons of 25% solution caustic soda had overflowed one of the bulk storage tanks and was contained within the containment system. The operator decided to add a small amount of dilution water and drain the containment, without obtaining the required permission from me, to the storm drainage ditch. The containment valves will be locked to prevent inadvertent operation. Eleven dead fish were noted along the entire ditch line. Sampling continued as well. I left Ms. Romanski a message at 1119. I called again at 1154 and spoke with Mr. Gendy, he stated that I needed to speak with Ms. Romanski on this matter. I spoke with Ms. Romanski on reporting requirements at 1315. She requested a detailed report of the incident with prevention techniques within five days. At 1330, pumping was stopped with another 93,750 gallons transferred out of the ditch from site #2.1 spoke with Ms. Romanski at 1433 and she stated that there were no public notification requirements, but we need to contact the EPA National Response Center since a Reportable Quantity was released. Final samples were drawn at 1512. The EPA NRC was contacted at 1600 and given report number 1082920. SPECIFIC cause(s) of the Bypass/Upset: During a heavy rain event, we were doing our best to prevent release of solids from secondary clarifiers. We had levels in aeration basins under control, but we experienced an unexpected sudden increase of incoming flow to the aeration basins that caused levels to rise rapidly. The water in the basin reached a level that caused water in the influent trough for the the basins to back-up and this allowed water to flow out of an opening for the basin air supply line and reach an area that has stormwater drains. The overflow was promptly discovered and action was taken to release additional flow from the aeration basins to the secondary clarifiers. The overflow lasted 20 minutes. Based on visual observation, flow was estimated to average 10 gal/sec for the first 5 minutes and then steadily slow down till it stopped completely. We estimate a flow of 5 gal/sec for the next 5 minutes, 2 gallons/sec for the next 5 minutes and 0.5 gal/sec the last 5 minutes. Total volume bypassed is estimated at 5,250 gallons. The total volume reached surface waters via storm water outfall #2. Due to the heavy rain event, Hominy Creek was already backing up into our outfall area. The increased water flow of the creek will quickly dilute the discharge. Also, due to the high levels of the creek it was not feasible to block the outflow of our stormwater outfall to the creek and attempt to contain for removal. The outfall area and downstream areas were monitored immediately after the event and thru Monday and no impacts to the stream or the environment were observed.
