Justin Henderson, of the Winston Salem Regional Office(WSRO)of the NC Division of Water Resources(DWR or the Division), conducted a compliance evaluation inspection of the single-family wastewater system at your residence on July 20,2017.The State requires the Division to inspect these types of systems every five years.An inspection report is attached for your records and the inspection findings are summarized below. 2. The system consists of a septic tank, filtration unit, and the discharge pipe. The system appears to be in good operating order, however the discharge pipe was unable to be located during this inspection. Effluent monitoring records were not available for review during this inspection. Please locate all components of your wastewater system and begin monitoring effluent for the parameters required by Permit Condition Part I (A.). 3. Septic tank solids removal records/receipts were not available for review during this inspection. Permit Condition Part I (Operation and Maintenance)(3.) states septic tanks shall be pumped out every five(5)years or when the solids level is found to be more than 1/3 of the liquid depth in any compartment, whichever is greater. These records are required to be retained for a period of 5 years per Permit Condition Part II Section D(6.). 4. The annual administering and compliance monitoring fee of$60 for 2016 is overdue. A Notice of Violation was issued on November 30, 2016 in response to this matter. Please remit payment to the following address: Attn.Animal/Discharge/Non-Discharge Billing, 1617 Mail Service Center Raleigh, NC 27699-1617 ASAP. Failure to pay the subject fee in a timely manner in accordance with 15A NCAC 02H .0105(b)(2) may cause the Division to initiate action to revoke the permit. Mr. Henderson left you with a copy of the most recent general permit. 5. The current permittee listed on the subject permit, Mack Johnson, is no longer the person legally responsible for the permit. Please update this information by submitting a Permit Name/Ownership Change Request application. This application was provided to you during this inspection. 6. Failure to properly operate and maintain your wastewater system can result in expensive repair costs as well as failure of the system to properly treat the wastewater.
