In accordance with your application for renewal of the subject Certificate of Coverage, the Division is forwarding
the enclosed General Permit. This renewal is valid from the effective date on the permit until July 31, 2002. This
permit is issued pursuant to the requirements of North Carolina General Statute 143-215 .1 and the Memorandum of
Agreement between North Carolina and the U.S. Environmental Protection Agency dated December 6, 1983. If any
parts,measurement frequencies or sampling requirements contained in this permit are unacceptable to you,you have
the right to request an individual permit by submitting an individual permit application. Unless such demand is
made, this Certificate of Coverage shall be final and binding.
The Certificate of Coverage for your facility is not transferable except after notice to the Division. Use the enclosed
Permit Name/Ownership Change form to notify the Division if you sell or otherwise transfer ownership of the
subject facility. The Division may require modification or revocation and reissuance of the Certificate of Coverage.
If your facility ceases discharge of wastewater before the expiration date of this permit, contact the Regional
Office listed below at [phone]. Once discharge from your facility has ceased, this permit may be rescinded.
This permit does not affect the legal requirements to obtain other permits which may be required by the Division of
Water Quality, the Division of Land Resources, Coastal Area Management Act or any other Federal or Local
governmental permit that may be required.

Dear Mr.Rottman:
The Division has received your invoice and letter requesting renewal of the subject permit
for your home wastewater treatment system. In your letter, you had some questions about the
permit and about the system.
The $240.00 fee is to renew the permit for 5 years. You will not be billed again until the
year 2002. There is no annual fee for this permit.
There is an annual testing requirement in the permit. When the permit is renewed, you
will be sent a new permit with all the requirements listed. There are laboratories which perform
the kind of tests that are required in the permit. Although we cannot suggest any one laboratory,
our regional office located in Winston-Salem should be able to help you find a lab in your area.

Enclosed please find some literature on home septic tank/sand filter systems. As stated
earlier, the Division will send you a new permit once it is renewed. The enclosed literature is a
general overview of home systems. The permit will contain the specific state requirements.
All homeowners whose homes utilize a septic tank/sand filter system with a discharge pipe
to a creek or other water body are required to have a permit just like yours to operate their
systems. There are several hundred homes across North Carolina with the same type of permit
you have. They,too,must meet the same requirements.

This letter to you is a letter requesting renewal of Permit#NCG550150 at 131 Springwood Dr.,
Eden, NC. 27288. Enclosed is a personal check for$240.00. I would appreicate if someone in
your office would answer the following questions I have in trying to understand what is expected
as a operator of a discharge or waste treatment facility.
First of all I want you to know that my wife and myself reside at the above address as a single
family residence. We purchased this home on June 22, 1996, and as of today, Feb.13, 1997, we
have spent approximately$500.00 for the following; name change on the permit from the
previous owner, water quality testing, chlorine tablets, and now a process fee for the renewal of =-)
the permit. -a •
Is this fee of $240.00 a yearly fee or a one time fee?
Is this permit for one or more years and is renewal required at the end of one or more years?
Is water quality testing required or suggested?
Does your office have any literature available to homeowners to lets us know what to expect
from your office?
Are all homeowners with sand filters required to have a permit and do we fall in the same
catagory with mobile home parks with sewage treatment systems?
As a new homeowner with a septic tank and sand filter system, I am begining to think I may
have made a mistake in purchasing this home and wish we would have bought one with just a
septic tank. If you can answer the above questions, I am usually home in the morning until 8:30
