Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between
North Carolina and the U.S. Environmental Protection Agency dated May 9, 1994 (or as subsequently
amended).
A minor error was corrected from the draft permit sent to you on February 4, 2004. The
Supplement to Permit Cover Sheet incorrectly listed the plant's capacity as 8.0 MGD instead of 0.05
MGD.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30)
days following receipt of this letter. This request must be in the form of a written petition, conforming to
Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings
(6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless such demand is made, this decision
shall be final and binding.
Please note that this pemut is not transferable except after notice to the Division. The Division may
require modification or revocation and reissuance of the permit. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or permits
required by the Division of Land Resources, the Coastal Area Management Act or any other Federal or Local
governmental permit that may be required.

hereby revoked. As of this permit issuance, any previously issued permit bearing this number
is no longer effective. Therefore, the exclusive authority to operate and discharge from this

Upstream/downstream samples shall be collected 3/Week during June, July, August, September, and 1 /Week
during the remaining months of the year.
Instream monitoring requirements shall be provisionally waived so long as the Permitee remains a member of
the Yadkin River Basin Association and the Association continues to function as approved by the Division and
Environmental Management Commission.
2. The monthly average effluent BOD5 and TSS concentrations shall not exceed 15% of the respective influent
value (85% removal).
3. Total Residual Chlorine monitoring is required only if chlorine is added to the effluent.
4. The pH shall not be less than 6.0 standard units nor greater than 9.0 standard units.
5. The daily effluent dissolved oxygen concentration shall not be less than 5.0 mg/1.
There shall be no discharge of floating solids or visible foam in other than trace amounts.

Summary: Union County requested the renewal of their NPDES permit on April 7, 2003. There have been no
modifications to the facility since the permit was issued in March 2002. The wastewater treatment plant serves
three schools in Union County.
Facility Description: The facility consists of influent pump station, bar screen, equalization basin, two aeration
basins, two clarifiers, aerated sludge digester, two tertiary filters, and UV disinfection with backup chlorination.
The capacity of the treatment plant is 50,000 gpd.
Waste Load Allocation (WLA): The last waste load allocation was performed in July 1996. BOD and
ammonia are water quality limited.
303(d) List: Crooked Creek is listed as impaired in the 2002 303(d) list due to biological data. The source of
the impairment is unknown. Potential sources are urban runoff and stormwater runoff.
Basin Plan: The use support rating is supporting for the portion of the creek where the discharge is located.
The benthos classification downstream from the discharge is Good -Fair; the classification upstream is Fair.
Ambient monitoring data for turbidity shows elevated values. Recommendations in the 2002 plan are to take
local action to reduce the effect of non -point source pollution, particularly from stormwater runoff as further
development occurs in the Crooked Creek watershed.
Pre -Treatment: No Significant Industrial Users.
DMR Review: Data were ' reviewed for the period of January 2002 to October 2003. Average flow was 0.01
MGD. Since this plant receives all the flow from the schools, during summer months flow is usually low or no
flow is reported. BOD averaged <2 mg/I and TSS <4 mg/l. Sampling results for weekly averages for fecal
coliform were greater than 400/100 ml on January, July, August, and September 2003. Other parameters
monitored are ammonia, dissolved oxygen, pH, total residual chlorine and temperature which were within
permit limits.

• Weekly average limits for ammonia were added to the permit as per the Ammonia Implementation
Policy. A weekly limit of 6.0 mg/I was implemented for the summer months and 12.0 mg/I for the
winter months.
• Monitoring for Conductivity was eliminated from the permit. The facility treats domestic waste
only, therefore monitoring for Conductivity is not necessary.
• The facility description was modified to include the equalization basin.

Management Commission implementing that Article, shall be guilty of a misdemeanor punishable by a fine not to exceed $10,000, or by

RESIDUALS MANAGEMENT PLAN
Grassy Branch WWTP
NPDES No. NCO085821
Union County, North Carolina
Residuals generated at the Grassy Branch WWTP will be transported via
vacuum truck to Union County's Crooked Creek WWTP (NC0069841). All
residuals at the Crooked Creek facility will be aerobically digested. The residuals
will be considered stable and therefore suitable for land application when the
following criteria are met:
1. At least seven (7) residuals samples will be collected and analyzed for Fecal
Coliform each monitoring period. The geometric mean of the densities of
these samples will be calculated and will be less than 2,000,000 Colony
Forming Units per gram of total dry solids prior to application.
2. Vector Attraction Reduction will be met through one of the following options:
a) volatile solids reduction of 38% or greater b) biosolids injection or
incorporation c) biosolids alkaline stabilization.
All residuals will be disposed of in accordance with Residual Land Application
Permit No. WQ0007486.
