5. Directions to Site: Take Hwy 109 South out of Winston-Salem
to the junction of Old Greensboro Rd (1756) at Ledford
Middle School. Turn left onto Old Greensboro Road and
travel approximately 5 miles to Saint Andrews Drive. Turn
left onto Saint Andrews and then right onto Cascade Drive.
The Freeze residence is on the left just past the first
pond.
6. Discharge Point(s) - List for all discharge points
Latitude: 350 58 ' 54" Longitude: 800 04 ' 56"
Attach a USGS Map Extract and indicated treatment
plant site and discharge point on map.

Although the lot is small and steeply sloped, space should
be available for repair of the system.
8. Topography (relationship to flood plain included) :
The sandfilter system is located adjacent to the flood
plain.

9 . Location of nearest dwelling: The Freeze residence is
surrounded by other dwellings in the subdivision.
10. Receiving stream or affected surface waters: U.T. to
Abbotts Creek
a. Classification: WS-III
b. River Basin and Subbasin No. : YAD-07
c. Describe receiving stream features and pertinent
downstream uses: The sandfilter system discharges into
a small pond on the Willow Creek Country Club golf
course that is tributary to Abbotts Creek. The
following facilities are located close to Mr. Freeze and
discharge into Abbotts Creek or its tributaries: Mr.
Alton Herndon (SFR NC0047635) , Mr. John Anderson (SFR
NC0070238) , Dr. Thomas Lauer (SFR NC0057550) , Mr. Gary
McClellan (SFR NC0057568) and the Willow Creek WWTP
(0. 08MGD NC0034452) . The Lake Thom-A-Lex water supply
reservoir is located approximately 6 miles downstream.
PART II - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS

existing) : The existing system c9nsists of the following:
1000 gallon septic tank, a 522 ft subsurface sandfilter,
chlorinator, contact chamber and outfall line.
5. Sludge handling and disposal scheme: Solids should be
removed from the septic tank by local pumpers for disposal
in the nearest POTW.
6. Treatment plant classification: NA. Less than 5 points

PART IV - EVALUATION AND RECOMMENDATIONS
The SFR system discharges directly into a small pond on the
Willow Creek Country Club golf course. Although the pipe is
submerged and cannot be sampled, the pond did not appear to be
suffering any adverse affects from the wastewater.
WSRO recommends reissuance.
