Landscaper's Maintenance Punch List
1. (Quarterly) Check the level of sediment accumulating in the inlet areas.
Removed sand grit, debris, trash and plant material by hand shovel at all inlet
points.
2. (Quarterly) Remove any debris, trash and collected sediment in the pipes that
blocks or slows down the flow of water into the stormwater device(s).
3. (Semi -Annual) Monitor the grassed swale to ensure that dense groundcover
continues to exist. Re -seed the area as needed and maintain at +/- 6 inches in
height.
4. (Annually) Monitor areas around the riser structures to ensure that settlement
has not occurred. Settlement may be a result of soil loss and/or pipe separation.
5. (Semi -Annual) Monitor ground cover on inside and outside pond slopes. Bare
or deteriorating areas around the ponds should be reseeded as needed to keep
area stabilized.
6. (Monthly) Make sure there are no obstructions around the riser structures.

On October 2, 2012, Robbie Bullock of the Washington Regional Office of the Division of Water Quality (DWQ)
inspected SECU-Swan Quarter in Hyde County to determine compliance with Stormwater Management Permit Number
SW7081009 issued on February 20, 2009. It was noticed during the file review that the Engineer's certification is not on
file as required by the permit and needs to be submitted to this office. A site inspection revealed that the site is compliant
with the terms and conditions of this permit. Please find a copy of the completed form entitled "Stormwater Inspection
Report" attached to this letter.
Please be advised that you are required to comply with the terms, conditions and limitations of your Stormwater
Management Permit under Title 15A North Carolina Administrative Code 2H .1003 and North Carolina General Statute
143-214.7, including operation and maintenance of your permitted stormwater system.
If the project has changed name, ownership or mailing address, a formal change of name/ownership form must be
submitted to DWQ within 30 calendar days detailing the change. Please provide the name, mailing address and phone
number of the person or entity that is now responsible for this permit.
Please be advised that violations of your Stormwater Management Permit may be subject to the assessment of civil
penalties of up to $25,000 per day per violation. If you have any questions please contact the me at the Washington

• The photo shows good establish vegetation on the embankments.
• The vegetation on the embankments should be trimmed [rice a year to
height of 12" to within 24" from the water edge. From the waters
edge and to within 24" along the berm the vegetation can be left to
grow wild.
• The outlet box is in good condition.

Comments:
• The photo shows good establish vegetation on the embankments.
• The vegetation on the embankments should be trimmed trice a year to
height of 12" to within 24" from the water edge. From the waters
edge and to within 24" along the berm the vegetation can be left to
grow wild.
• The outlet box is in good condition.

The photo above shows the transition berm between forebay and the
main pond body. It was found to be in good condition with adequate
grass cover.
