5. Directions to Site: From Winston-Salem proceed south on Hwy
109 and turn left onto Old Greensboro Road (SR 1756) at
Ledford Middle School. The Herndon SFR is located on the
left side of SR 1756, approximately 0. 7 mile east of the
intersection of SR 1757 and SR 1756.

downstream uses: The WWTP system discharges into a dry
ditch. A pond in Willow Creek Country Club is located
800 ft downstream of the discharge.

4 . Alternative Analysis Evaluation: Has the facility evaluated
all of the non-discharge options available. Please provide
regional perspective for each option evaluated.
Spray Irrigation or Subsurface: Soil characteristics in the
area are unsuitable. This is why a discharging system was
installed originally.
Connection to Regional Sewer System: No municipal sewer
located within a reasonable distance.
PART IV - EVALUATION AND RECOMMENDATIONS
Previous inspections have found the system to be operating
properly. A tablet chlorinator and contact chamber were added to
the system in 4/87 . The WWTP system discharges into a brier
patch on the side of a hill approximately 50 feet from the zero
flow ditch below Mr. Herndon's dam for his 1. 5 acre pond. The
entire backyard (lake and discharge point included) is fenced to
control access. It is also relevant to note that another pond in
the Willow Creek Country Club subdivision is located 800 ft
downstream of the outfall.
WSRO recommends reissuance.
