The Division of Water Quality has received the sampling reports you have submitted on behalf of the subject
facility. We would like to take this opportunity to request that you modify your current reporting practices.
Results of monitoring are to be reported on standardized, approved Discharge Monitoring Report (DMR) forms.
These DMR forms have space for providing facility information as well as characteristics of the discharge. They
are typically prepared as a two-sided form with data on one side and the permittee's certification of compliance
status on the back. Under most circumstances, the double sided DMR form is all a facility needs to submit to
document the quality of an outfall's effluent during a particular reporting month. Support information (like a
copy of the permit, lab results, chains of custody, etc.) does not need to be submitted along with the DMR. Such
records are required to be retained by the permittee for at least three years and must be made available to Division
personnel upon request. Submitting these extra records along with DMRs generally does not add any value to the
information already provided on the form, and takes up additional space in our records retention areas.
A copy of the DMR form typically used by facilities such as this one is attached along with a discussion regarding
how to complete the form. All DMRs are to be submitted to our Central Files address as indicated on the DMR

Hi guys,
I issued this permit for a groundwater remediation system in July 2006. Since then,
every month I've received a large, bound document titled "NPDES Sampling Report". I
assumed the consultant was sending me a copy out of an overabundance of caution.
From time to time I've checked BIMS and they haven't had any violations, so I've sent
them down to Central Files. Back in February I realized that not only were they not
getting any violations- there was no data recorded in BIMS at all! I called the
consultant and asked if they were submitting DMRs. They weren't aware that they
needed to. I explained that we needed DMRs and sent them a link to the website.
They said they'd quickly fill out all the back DMRs and get them in ASAP. Still,
every month I get the sampling reports and send them down to Central Files. Today I
got another and decided to check BIMS. Still, no DMRs, no violations. I was about
to send another note to the consultant, but while flipping through the sampling
report I noticed that in the back they have appended each and every DMR since July
2006.
According to the sampling report, they have been discharging since September 2006.
They probably never submitted an engineers certification because for several months
before they received the permit, they had a temporary permit from CMUD to discharge
their wastewater into the sanitary sewer. So the facility was already operating
before they got the permit.
Can you guys sort this out? There must be a stack of these reports in central files
now, plus they've been doing their quarterly WET as required. t
