Per 15A NCAC 02T.0105(c)(6), directly related environmental permits or certification applications must be being prepared,
have been applied for, or have been obtained. Issuance of this permit is contingent on issuance of dependent permits (erosion
and sedimentation control plans, stormwater management plans, etc.).

Per 155A NCAC L22T_0402, "high -priority sewer" means any aerial sewer, sewer contacting surface waters,
siphon, or sewers positioned parallel to streambanks that are subject to erosion that undermines or deteriorates the sewer.
Siphons and sewers suspended through interference/conflict boxes require a variance approval.

High priority lines shall be inspected by the permittee or its representative at least once every six -months and
inspections documented per 15A NCAC 02T.0403(a)(5) or the permittee's individual System -Wide Collection permit.

Dear Mr. Simpson:
On behalf of the City of Wilson, please find attached Request for Modification package for the
5114 Packhouse Road (SR 1382) project. The force main length and location has been extended by 455
linear feet, to allow discharge into a different manhole on Hunting Ridge Road. As before, the City will
operate and maintain this force main as part of their collection system.
Please find attached two (2) copies of each of the following documents for your review and
approval:
• Fast -Track Sewer Application Form — One (1) original and one (1) copy
• 8 1/2" x I I" copy of project location on USGS Quadrangle Maps
• 8 1/2" x I I" copy of project location on NRCS Soils Mapping
Please find attached check in the amount of $600.00 to cover the Request for Modification fee.
