Program Information Comment E: 1. This program also includes the operation of temporary earthen storage structures and evaporation ponds permitted in the Attachment B Certifications approved on September 10, 2019 and July 13, 2023. The program information shall be updated to include the operation of these structures as a part of this permit. Diamond Grinding Slurry residuals will be recovered and stored within mobile storage units or approved residuals storage facilities at or near each road project that is scheduled for diamond grinding. Currently two modifications for temporary earthen storage of have been approved for the existing permit term. The storage basin for the I-3802B in China Grove area, Rowan County has been closed out. See Attachment A for location, storage modification approvals, and status imagery of the I-3802B prior basin location. The approved storage modification basin for I-3819B/U-6039 in Statesville, Iredell County is the process of being closed out. See Attachment B for location, storage modification approvals, and status imagery of the I-3819B/U-6039 basin location. At both locations a primary receiving basin stored the bulk of the materials. Surface/floating pumps were used to lift the liquid/water fraction to elevated basins where the remnant water was returned to the grinding operation or allowed to evaporate. Solids, exhibiting concrete properties, were left in place and utilized as beneficial fill for the construction of the transportation projects. HOS will be recovered and stored within mobile storage units. Typically, HOS residual volumes do not require the use of approved earthen residuals storage facilities. However, if a need for earthen storage facilities arises, the facility will be permitted at or near each bridge project that is scheduled for hydrodemolition and will follow the plan requirements as detailed for DGS residuals storage facilities. FORM DCAR 06-16 (ITEM H. – Operation and Maintenance Plans) Comment F: 1. See Item E.1. The O&M Plan shall be updated to describe the operation of these earthen storage structures in the program. 2. The program information for the hydrodemolition operation slurry and diamond grinding slurry programs mention the use of mobile storage units as referenced in permit Condition III.8. The O&M Plan shall be updated to describe the operations of these storage structures for this program. The O&M plan has been updated to include temporary mobile and earthen storage (see Attachment C) with the changes in blue text for ease of review. FORM RSC 06-16 (ITEM B. – Residuals Source-Generating Facility Information) Comment I: 1. Process Flow Diagram & Narrative, See Comment E.1. All earthen storage units will be individually permitted as a modification by NCDEQ Non-Discharge Branch per previous discussions with Non-Discharge Branch staff. The Contractor will be responsible for determining the need for earthen storage in order to complete the concrete diamond grinding. The Contractor will submit a preliminary plan to the NCDOT, which upon acceptance will be submitted to the Non-Discharge Branch for review and approval. The modification submittal shall be prepared in accordance with the requirements of form DCAR06-16, specifically items E and F. Once approved, the Contractor shall operate the storage basin in accordance with the requirements and conditions stipulated in the permit modification. Routine inspection and maintenance will be performed in accordance with the operation and maintenance plan included in the modification submittal or as amended by the approved permit modification. Remnant water may be returned to the grinding operation. Any discharge of DGS or remnant water will be done in accordance with permit WQ0035749. Upon completion of the diamond grinding operation, any remaining DGS or remnant water will be land applied under WQ0035749 or taken to a permitted facility for disposal. Any material passing EPA Test Method 9095B Paint Filter Liquids Test shall be considered solid waste and shall be disposed of in accordance with Solid Waste Rules. The site shall be graded to blend to existing ground contours or brought to finished plan elevations and appropriately stabilized with vegetation if not located under proposed paved surfaces FORM RSC 06-16 (ITEM C. – Residuals Quality Information) Comment L: 1. Item C on the RSC 06-16 form requests the laboratory analytical reports and operational data used to complete this certification. Please provide the most recent calcium carbonate equivalence (CCE), percent total solids, and pH for a DGS and an HOS project, respectively. The most recent DGS analysis for calcium carbonate equivalence (CCE), percent total solids, and pH is attached (see Attachment D). HOS land application was not performed during this permit term and no analysis is available. Gauges to monitor waste levels in the temporary storage structures shall be provided. These gauges shall have readily visible permanent markings, at inch or tenth of a foot increments, indicating the following elevations: maximum liquid level at the top of the temporary liquid storage volume; minimum liquid level at the bottom of the temporary liquid storage volume; and the lowest point on top of the dam. A protective vegetative cover shall be established and maintained on all berms, pipe runs, erosion control areas, surface water diversions, and earthen embankments (i.e., outside toe of embankment to maximum allowable temporary storage elevation on the inside of the embankment). Trees, shrubs, and other woody vegetation shall not be allowed to grow on the earthen dikes or embankments. Earthen embankments shall be kept mowed or otherwise controlled and accessible. Freeboard (i.e., waste level to the lowest embankment elevation) in the temporary storage structures shall be measured to the nearest inch or tenth of a foot, and recorded weekly. Department of Transportation Permit No. WQ0035749 DIAMOND GRINDING SLURRY & HYDRODEMOLITION OPERATION SLURRY I. Operational Functions Hydrodemolition Operation Slurry Description and Overview The hydrodemolition operation slurry (HOS) is generated by the hydrodemolition of concrete on bridge decks during the deck restoration process. The hydrodemolition operation will typically generate a range of volumes per bridge with percent solids of the unfiltered water ranging from 0.5 to 10 percent. The hydrodemolition operation will typically generate a range of approximately 10,000 to 50,000 gallons of HOS per bridge. The hydrodemolition process shall be managed, monitored, collected, and properly disposed of as a portion of the bridge deck restoration process. Many variables will determine the quantity, such as hydrodemolition contractor process, size of bridge deck, total depth of concrete to be removed, etc. The generated water and solids (slurry) is stable and contains a high calcium value for application as a lime amendment. Diamond Grinding Slurry Description and Overview The diamond grinding slurry (DGS) is generated by diamond grinding of concrete pavement which generates a slurry material consisting of water and pulverized concrete. This material is collected in trucks through a vacuum system concurrent with the grinding process. The generated slurry is stable and also contains a high calcium value for application as a lime amendment. The diamond grinding process shall be managed, monitored, collected, and properly disposed of. The diamond grinding of concrete will generate a wide range of slurry volume per project. Many variables will determine the quantity, such as contractor performing the diamond grinding, length of roadway, etc. The HOS and DGS shall be collected, hauled, and discharged in accordance with local, state, and federal regulations. The HOS and DGS will not be allowed to enter storm sewers, bridge drainage downspouts or bridge approach downspouts, ditches, surface waters, floodplains or wetlands. The HOS and DGS will be monitored to confirm its compliance with conditions in Non-Discharge Permit No. WQ0035749 to include: • Calcium Carbonate Equivalence (CCE) • Percent Total Solids • pH Temporary storage of DGS/HOS shall conform the requirements of WQ0035749. Temporary mobile storage units shall be water-tight tankers or tanks located on the project or in the adjacent vicinity. It is the Contractor’s responsibility to obtain a permit modification for excavated/earthen DGS/HOS storage facilities from NCDEQ prior to construction of the storage facility. These permitted storage facilities will be located on NCDOT property adhering to the offsets and conditions stipulated in the modification. The contractor shall grade and revegetate the site upon basin closure. When land application of HOS is not a feasible option, the primary method of disposal will be to transport the HOS to a local publicly owned treatment works (POTW) or a licensed treatment or disposal facility. When land application of DGS is not a feasible option, other methods of disposal will be determined such as transport of the liquid portion to a local publicly owned treatment works (POTW) or a licensed treatment or disposal facility and disposal of the solid portion in an approved solid waste disposal facility. II. Maintenance Schedules Maintenance will be the responsibility of the Contractor performing the hydrodemolition or diamond grinding operations. In addition, subcontractors must have a current Land Application of Residuals "Operator in Responsible Charge" (ORC) as certified by the North Carolina Water Pollution Control System Operators Certification Commission (WPCSOCC). III. Safety Measures Safety plans and training will be the responsibility of the Contractor performing the hydrodemolition or diamond grinding operations. All subcontractors will be required to comply with safety plans and training as applicable to the Contractor's performing the hydrodemolition or diamond grinding operations. NCDOT recognizes that the safety of employees and contractors is paramount to the maintenance and construction of the state transportation system. Therefore, NCDOT recognizes no boundaries in the development and implementation of a world class safety and health program that will protect their employees, contractors and the traveling public. The NCDOT Safety Philosophy is as follows: • All accidents and injuries can be prevented. • Management/supervisors are responsible, and will be held accountable, for preventing injuries and occupational illnesses. • Occupational safety and health is part of every employee's total job performance. • Working safely is a condition of employment. • All workplace hazards can be safeguarded. • Training employees to work safely is essential and is the responsibility of management/supervision. • Prevention of personal injuries and accidents is good business.
