b. The inspector could not locate the daylight of the pipe at the highway.
c. The catch basin nearest the road does not appear to catch the drainage area.
d. There are two areas where additional impervious area has been constructed. The areas
circles below contain impervious area that is not on the permitted plans. A modification is
needed to reflect the constructed condition.

1. Your permit expired on April 1, 2007 and a renewal request has not been received to date. This is a noncompliant condition
and should be addressed immediately.
2. A signed copy of the Engineer's certification has not been provided as required. This must be submitted at once along with
your renewal request.
3. The forebay area should have the elevations checked to be sure that it has not filled with to much sediment. If required,
sediment must be removed so that the pond can work as designed.
4. The grass was moderately high on the sideslopes and should be mowed as required in your operation and maintenance
agreement.
5. Pond levels are low due to near drought conditions.
