Sullins, Director Division of Water Quality Wake County EXP# 06-1139v2 On December 17, 2007, the Express Review Program of the Division of Water Quality (DWQ) received your application for the above referenced project. The DWQ has determined that your application will require additional information. Please provide the following information so that we may continue to review your project and prevent denial of your application as required by 15A NCAC 2H .0506: Utility line impact: a. Please provide the proposed temporary impact to the perennial stream and riparian buffers at the sewer line impact site. The written description mentions 23.2 linear feet of streambed, while the table is blank in the PCN with side notes of 15 and 17.5 linear foot references. Additionally, Zone 1 impacts are written as 1,415 sq. feet in Section VI-1 and as 1,067 sq. feet in the table while the Zone 2 riparian buffer impact is shown as 960 sq. feet in Section VI-1, 920 sq. feet on the table, and 720 sq. feet on the submitted plan sheets. b. Please provide EEP acceptance letter for Zone 1 riparian buffer impacts that are required when an "other than perpendicular crossing" is proposed. c. Please provide cross sectional construction details of the proposed impact site (showing the required burial depth, etc.). 2. Grassed Swale: a. Please provide a BMP Supplement Form for the proposed grassed Swale. The BMP supplement Forms are available at http://h2o.enr.state.nc.us/su/bmp forms.htm. Please include both the Design Summary and the Required Items Checklist along with all the required items listed therein. b. Please include a signed and notarized Inspection and Maintenance Agreement Form for the proposed grassed swale. c. Please size the swale for both onsite (road and adjoining lands) and offsite (west portion of Swale) drainage since the grassed Swale appears to receive runoff from some portion of the land to the west. d. Please provide supporting calculations for estimating the flows and velocities indicated on the plan sheet 3. e. Please select one of the grasses specified in Table 2 of the Grassed Swale Chapter of the North Carolina Best Management Practices Manual that will be suitable for the site, instead of Centipede sod as specified on plan sheet 3). £ Construction Sequence shall specify how the grassed swale (and any other proposed BMP) shall be installed.
