North Carolina General Statute 143.215.1(c) requires that an application for permit renewal be filed in a timely manner. If continuation of the permit is desired,please submit the following information within 30 days of receipt of this letter: 1. A letter requesting the renewal. 2. The completed invoice form(copy attached). 3. A processing fee of$240. The check should be made payable to the North Carolina Department of Environment, Health, and Natural Resources which may be abbreviated as NCDEHNR. Failure to request renewal within this time period may result in a civil assessment of at least$250.00. Larger penalties may be assessed depending upon the delinquency of the request. If operation of a discharge or waste treatment facility is to occur after the expiration date of the permit, or if continuation of the permit is desired,it must not be allowed to expire. Operation of the waste treatment works or continuation of a discharge after the expiration date would constitute a violation of NCGS 143-215.1 and could result in assessment of civil penalties of up to$10,000 per day.
