Please read this report carefully and take note of any comments that are listed. Please review the enclosed NPDES NCG550000 General Permit and ensure compliance at all times. You will also find enclosed a technical bulletin that provides an overview of your permit type. Although your system may only receive low intermittent flow, as it is not occupied year-round, it is considerably old and the septic tank should be checked and pumped if necessary to prevent solids carrying over into the sand filter. • Septic tanks and secondary tank (recirculating/pump tanks) shall be inspected at least yearly to determine if solids must be removed or if other maintenance is necessary. Septic tanks shall be pumped out every five years or when the solids level is found to be more than 1/3 of the liquid depth in any compartment, whichever is greater. All tanks should be emptied of their contents whenever any of the tanks meet this requirement. Effluent filters (if present) shall be inspected at least once a year, cleaned and reinstalled or replaced. Contents removed from septic tanks shall be disposed at a location and in a manner compliant with all local and state regulations. Page# 1 1,1 E 4-- 3 NPDES yr/mo/day Inspection Type (Cont.) 1 31 NCG550125 Ill 121 16/03/30 117 18 [ Section D:Summary of Finding/Comments(Attach additional sheets of narrative and checklists as necessary) On March 30, 2016, Rob Topolski and Landon Davidson of the Asheville Regional Office(ARO) conducted a compliance evaluation inspection of the Single Family Residence wastewater treatment system (NPDES Permit No. NCG550125) at 221 Aaron Church Lane in Avery County, NC. The septic tank and the effluent pipe could not be located. Drainage pipes from two flowing springs(one used as a drinking water source)were observed across the road from this system where the effluent pipe would likely discharge to. No stream impacts from this system were observed. The property li owner/permittee, Mr. Wayne Gilbert was not present during this inspection; however, he was contacted by phone on 4/13/16. He said that he only uses the residence for a short period each year and that he has never had the septic tank pumped.
