preliminary review of your renewal permit application package. Additional information is required
before we may continue our review. Please address and submit the following item(s) within 30 (thirty)
days of receipt of this letter:
1. Waste Utilization Plan: The WUP dated Nov 11, 2020 includes a lease agreement for fields MG1
— MG5. That lease agreement was only valid for Nov 1, 2020 through Nov 1, 2021. Please
provide an updated lease agreement or and update WUP with these fields removed.
Please reference the subject application number when providing the requested information. All revised
and/or additional documentation shall be signed, dated and sent to my attention at the address below or
