NC Department of Environmental Quality (DEQ) — Division of Water Resources (DWR) NOTIFICATION OF INTENT (NOI) TO CONSTRUCT OR OPERATE INJECTION WELLS The following are `permitted by rule" and do not require an application to be submitted and an individual permit be issued when constructed in accordance with the rules of 15A NCAC 02C.0200 (NOTE: This form must be received at least 14 DAYS prior to injection) GROUNDWATER (GW) REMEDIATION INJECTION PERMIT TYPES: (1) IN -SITU GW REMEDIATION PERMIT TYPE (15A NCAC 02C .0225) • In -Situ Passive Injection Systems - In -well delivery systems to diffuse injectants into the subsurface. Examples include ORC socks, iSOC systems, and other gas infusion methods (Note: Injection Event Records (IER) do not need to be submitted for replacement of each sock used in ORC systems). • In -Situ Small -Scale Injection Operations — Injection wells located within a land surface area not to exceed 10,000 square feet (SF) for the purpose of soil or groundwater remediation or tracer tests. If area to be treated exceeds 10,000 SF do not use this NOI; an injection Permit application shall be submitted, and a Permit issued per 15A NCAC 02C .0225(f). • In -Situ Pilot Tests - Preliminary studies conducted for the purpose of evaluating the technical feasibility of a remediation strategy to develop a full-scale remediation plan for future implementation, and where the surface area of the injection zone wells is located within an area that does not exceed five percent of the land surface above the known extent of groundwater contamination. Also, if pilot test is going to be conducted on separate groundwater contaminant plumes do not use this NOI; a Permit application shall be submitted, and a Permit issued as per 15A NCAC 02C .0225(f). • In -Situ Thermal (IST) — IST wells `heat' contaminated groundwater in -situ to enhance remediation. (2) AIR INJECTION PERMIT TYPE (15A NCAC 02C .0225) These permit types are used to inject ambient air to enhance treatment of soil or groundwater. (3) TRACER WELL PERMIT TYPE (15A NCAC 02C .0229) These permit types are used to inject substances for determining hydrogeologic properties of aquifers. (4) AQUIFER TEST PERMIT TYPE (15A NCAC 02C .0220) These permit types are used to inject uncontaminated fluid into an aquifer to determine aquifer hydraulic characteristics. L DESCRIPTION OF PROPOSED INJECTION ACTIVITIES AT THE SITE — Provide a brief narrative regarding the cause of the contamination, and purpose, scope, goals of the proposed injection activity: Five documented historical petroleum releases have contributed to soil and groundwater contamination in and around the diked area surrounding several above ground storage tanks (ASTs). Operation of the biosparging system since June of 2008, and a system expansion in June 2013 to include additional biosparging wells on the southern portion of the property have made significant progress in reducing contaminant concentrations throughout the site. To address remaining groundwater contamination near the southern property boundary and allow conditional NFA status, additional focused corrective actions will be conducted around MW-8, and MW- 19 and MW-21. Low concentrations of 1,2 dichloroethane (1,2-DCA) near MW-8 and methyl tertiary butyl ether (MTBE) near MW-19 and MW-21, slightly exceed their respective North Carolina Groundwater Quality Standards (NCGWQS). The proposed focused corrective actions will consist of subsurface injections of Oxygen BioChem (OBC) using a direct push technology DPT) rig. OBC is a NCDEQ-approved additive and consists of sodium persulfate (oxidant) and calcium peroxide which activates the persulfate and provides long-term oxygen release to promote aerobic biodegradation. Background Five documented historical petroleum releases have contributed to soil and groundwater contamination in and around the diked area surrounding several above ground storage tanks (ASTs) (Figure 1). In 2006, following completion of a Comprehensive Site Assessment (CH2M, 2006a) a CAP was approved by the North Carolina Department of Environmental Quality (NCDEQ, formerly North Carolina Department of Environment and Natural Resources) to implement a biosparging remedy (CH2M, 2006b) (Figure 2). Operation of the biosparging system began in June of 2008, and a system expansion occurred in June 2013 to include additional biosparging wells on the southern portion of the property. In April 2016, NCDEQ suspended the requirement to perform corrective action at the Site, and the risk classification for the Site was reduced from 'high' to 'intermediate' (NCDEQ, 2016). The intermediate ranking was based on the suspected presence of petroleum -impacted soil exceeding the industrial/commercial maximum soil contaminant concentrations (MSCC). The biosparging system remained offline through March 2018 when it was reactivated in response to a slight increase in contaminant concentrations near MW-14 and MW-17. Jacobs Engineering Group Inc. .;acobs April30, 2024 Subject: Corrective Action Plan - Addendum In September 2023, following a limited soil investigation that showed soil contamination had attenuated below the industrial/commercial MSCCs, NCDEQ approved the site risk classification to 'Low'. NCDEQ also recommended use of a Notice of Residual Petroleum (NRP) and land use restrictions to obtain conditional No Further Action status for the remaining soil that exceeded residential MSCCs. Corrective Action To address remaining groundwater contamination near the southern property boundary additional focused corrective actions will be conducted around MW-8, and MW-19 and MW-21 (Figure 3). Low concentrations of 1,2-dichloroethane (1,2-DCA) near MW-8 and methyl tertiary butyl ether (MTBE) near MW-19 and MW-21, slightly exceed their respective North Carolina Groundwater Quality Standards (NCGWQS). The proposed approach will involve subsurface injections, as described below. Injection Plan The proposed focused corrective actions will consist of subsurface injections of Oxygen BioChem (OBC) using a direct push technology (DPT) rig (Figure 3). OBC is an NCDEQ-approved additive and consists of sodium persulfate (oxidant) and calcium peroxide which activates the persulfate and provides long-term oxygen release to promote aerobic biodegradation. MW-8 DPT injections would be performed around MW-8 in a 10-ft by 10-ft grid pattern with the downgradient points located as close as possible to the property boundary/fence line (Figure 4). MW-19 and MW-21 DPT injections would be performed around MW-19 in a 10-ft by 10-ft grid pattern with the downgradient points located as close as possible to the property boundary/fence line. Additional DPT injections would also be performed around MW-21, located hydraulically upgradient (inside the tank dike), in two parallel rows of injections on 10-ft centers, oriented perpendicular to groundwater flow direction (Figure 5 and 6). OBC Delivery At the three locations, injections would be conducted within the water table at 1-ft vertical intervals to a maximum depth of approximately 25 ft bgs or refusal, whichever is shallower. Approximately 230 lbs. of OBC will be injected at each DPT point. The attached "Notification of Intent to Construct or Operate Injection Wells" (NOI) provides specific details of the injection process. Performance Monitorina Groundwater monitoring would be conducted using MW-8, MW-19, MW13, and MW-21 beginning three months after injections. Sampling procedures will be conducted in accordance with the 2006 CAP. Operation of the biosparging system since June of 2008, and a system expansion in June 2013 to include additional biosparging wells on the southern portion of the property have made significant progress in reducing contaminant concentrations throughout the site. To address remaining groundwater contamination near the southern property boundary and allow conditional NFA status, additional focused corrective actions will be conducted around MW-8, and MW- 19 and MW-21.
