Susan, Please see the attached documents. Please note that the internal road network shown on the aerial map and the USGS does not reflect the current approved road network. The current road alignment and some of the road names have changed from what was approved by a previous developer. A few questions/comments: We'll accept this older application form (FTA 06-21), but please use FTA 10-23 in the future. You can find the new form at the link: https://www.deq.nc.gov/about/divisions/water- resources/permitting/municipal-npdes-pretreatment-and-collection-system/collection- systems/sewer-extension-permitting (and also the updated FTSE form FTSE 10-23) 2. Part 11.4. Please re -check and revise the [at/long for this project. 3. Part V1.7. Louisburg can use the 75 GPD/Bedroom minimum design criteria (based on new legislation passed October 2023). Please revise this part and the FTSE page. I think at the time of initial review of the original submittal— it was not clear that folks could use the 75 GPD/BR minimum design criteria yet. 4. Why does Part V1.8. not match Part VI.7? I assume this is being constructed as one complete project (therefore, those figures should be the same). 5. Again, via your application, you have indicated and you are ensuring that this project does comply with the setbacks set forth by the Tar Pamlico state buffer rules. You have also indicated that this project does not require coverage/authorization under a 404 or 401 permit. Please reconfirm. 6. The maps submitted are not sufficient (and this was pointed out in the previously returned submittal). Please provide a topo and aerial map that clearly identifies the project area, including the closest surface waters, general location of the gravity sewer (and downstream pump stations if applicable) and downstream connection point for the receiving sewer. (An example is attached above). Thanks much. Let me know if you have any questions.
