I have made the corrections to the facility names for these in our system. I have also corrected the renewal COCs,
which are attached and now saved in our repository permit files.

I see where the facility names for either were not updated correctly or completely back in 2022-one of them was
just a mix-up I think. The form for NCG1700126 does indicate the new facility name should be Valdese Weavers
LLC(Lovelady Road Plant), but the form for NCG170073 says the facility name should have been the same as the
new owner name,just Valdese Weavers LLC.That was maybe inadvertent, based on the other requests the came
in at the same time,so I've updated the Perkins Road Plant to include that in the name.
There was an email sent on 8/10/2022 with summary reports to confirm the changes for all sites, but the errors for
these two must have been missed.You can run a Permit Summary Report for these two permits tomorrow
morning to verify everything is now correct(the report takes 24-48 hours to refresh with the changes I
entered today).

I am helping Valdese Weavers renew the COCs for their 4 plants located near Valdese, NC.Two of them have the
correct permit summary facility information and two of them have the incorrect facility names. Can you please

Why is this information needed?
Regulations in 40 CFR §122.63 allow for minor modifications to NPDES permits for a change of
ownership or operational control of a facility, provided that information supports that no other change
in the permit are necessary.
Why does this form need to be mailed in?
Permittees and applicants must fulfill signatory requirements in the NPDES federal
regulations in 40 CFR §122.22 (please see those regulations for guidance). Until NCDEQ's
electronic submission process meets Cross-Media Electronic Reporting (CROMERR)
requirements, this original signed (not digital signature) form must be mailed to the
address below. The uploaded copy is stored as part of the permit record in the Division's
digital repository.
