We find the plan to be acceptable with modifications and hereby issue this letter of Approval with Modifications. The Modifications Required for Approval are listed on the attached page. This plan approval shall expire three (3) years following the date of approval, if no land-disturbing activity has been undertaken, as is required by Title 15A NCAC 4B .0129. As of April 1, 2019, all new construction activities are required to complete and submit an electronic Notice of Intent (eNOI) form requesting a Certificate of Coverage (COC) under the NCG010000 Construction General Permit. After the form is reviewed and found to be complete, you will receive a link with payment instructions for the $120 annual permit fee. After the fee is processed, you will receive the COC via email. As the Financially Responsible Party shown on the FRO form submitted for this project, you MUST obtain the COC prior to commencement of any land disturbing activity. The eNOI form may be accessed at deq.nc.gov/NCG01. Please direct questions about the eNO1 form to the Stormwater Program staff in the Raleigh central office. If the owner/operator of this project changes in the future, the new responsible party must obtain a new COC. Title 15A NCAC 4B .0118(a) and the NCGO1 permit require that the following documentation be kept on file at the job site: 1. The approved E&SC plan as well as any approved deviation. The NCGO1 permit and the COC, once it is received. 3. Records of inspections made during the previous 30 days. Also, this letter gives the notice required by G.S. 113A-61.1(a) of our right of periodic inspection to ensure compliance with the approved plan. North Carolina's Sedimentation Pollution Control Act is performance-oriented, requiring protection of existing natural resources and adjoining properties. If, following the commencement of this project, it is determined that the erosion and sedimentation control plan is inadequate to meet the requirements of the Sedimentation Pollution Control Act of 1973 (North Carolina General Statute 113A-51 through 66), this office may require revisions to the plan and implementation of the revisions to ensure compliance with the Act. Acceptance and approval of this plan is conditioned upon your compliance with Federal and State water quality laws, regulations, and rules. In addition, local city or county ordinances or rules may also apply to this land- disturbing activity. This approval does not supersede any other permit or approval. Please note that this approval is based in part on the accuracy of the information provided in the Financial Responsibility Form, which you provided. You are requested to file an amended form if there is any change in the information included on the form. Adequate measures(e.g. silt fence, straw wattles,etc.)shall be provided below all proposed land disturbance activities where disturbed areas flow towards property owned by others and/or streams/drainage channels. In some locations, there are no proposed measures located below the land disturbance activity to contain sediment. Some examples are: Sheet C-1.01 from STA 10+50 to near STA 11+40. Sheet C-1.02 on South side of N. Camden Rd. and North side of McIntyre Rd. Sheet C-1.03 from STA 38+00 to near STA 39+75. Sheet C-1.05 North of STA 75+96. Adequate inlet protection shall be provided for drop/yard inlets and/or open pipe inlets which receive runoff from disturbed areas. 2. Response letter indicated design calculations were attached, however, the design calculations were not received. Provide design calculations for stabilization of ditch lines located throughout the site. Ditch lines with a velocity of 2 fps or greater require a temporary liner. Ensure the permissible velocity in grass lined channels are not exceeded. Ensure the type of liner to be installed is identified(e.g.NAG S75, Curlex 1, or approved equal, etc.). This information must be provided to this office prior to the start of land disturbance activities. 3. Sheet C-1.03 Comments: The line installation at STA 46+60 will cross a stream. All line installation must be conducted in the dry. Provide the minimum temporary culvert size necessary to convey bank-full flow and an adequate construction sequence of how all work will be conducted in the dry. Ensure stream channel is properly stabilized prior to removing coffer dam and temporary culvert. An adequate outlet in the silt fence must be provided where silt fence is shown crossing the existing ditch line near STA 47+60. In addition, adequate channel stabilization must be provided where runoff from storm drainage system flows across LOD near STA 47+60 to STA 48+20. The minimum temporary stream culvert and construction sequence must be provided to this office prior to the start of land disturbance activities. The print quality of Detail#7 (Sheet C-2.02) is barely visible for some areas. Ensure print is clearly visible. 4. Sheet C-1.04 Comments: An adequate outlet in the silt fence must be provided where silt fence is shown crossing the swale area near STA 52+50. Provide excavated storage area as needed upstream of the silt fence outlet. Provide adequate outlet protection below the culvert outlet from the road.
