Hello Demetri and David,
After reviewing the Fast Track Sewer System Extension Application for South Main Office, additional
information is required in order to further process the application. Please provide the following information
by July 13, 2024, and note that the requested information must be submitted within 30 days of this request, or
the applicant will be required to submit a new application and application fee in accordance with 15A NCAC 02T
.0107.
1. Is this an ARPA funded project? If soplease include the DWI assigned ARPA project number in
your response.

2. Section 1.5 of the FTA 10-23: Please verify if the email address '[e-mail]'
and the phone number are correct. Also, ensure that they are addressed to the signatory
authority mentioned in Section 1.3 of the FTA 10-23.
This was as intended. However, to make things less confusing, the contact information has been
changed to that of the Signature Authority's name (See attached). As the managing member of
the LLC, Dr. Economedes does have the authority to sign the application.
3. Section IV.1 of the FTA's facility name does not match with the facility name registered for permit
NC0063096.
The image below was taken directly from the permit so not sure what the discrepancy is. I'm
happy to call it something else if you'll tell me what you want to call it.

4. Section V1.7 of the FTA is missing information about the establishment type and the generated
flow from this project. See 02T .0114 (f) for further classification. Additionally, section V1.8 should
mention '0' if the flow has already been allocated.
As this is a 0 flow permit, as the flow has already been allocated, Section IV.7 was not
completed. The flow has now been noted as 0 in Section V1.8 (See attached).
5. Section V111.5 of the FTA mentioned about 2" PVC pipe for the force main. Please provide details of
the material type as the 02T .0305 & MDC (Pump Stations/Force Mains).
The pipe material is 2" ASTM D2241 SDR 21 PVC (See attached).
6. According to Section F of the Application Instructions, an 8.5-inch x 11-inch color copy of a USGS
Topographic Map and an aerial map should be included with the application. The maps need to
clearly identify the project area, including the closest surface waters, general location of the
gravity sewer, pump stations, and force mains, and the downstream connection points for the
receiving sewer. Please provide the required maps according to the application requirements.
See revised maps (attached).

7. As the map was unclear, however, Section IX.5 of the FTA was checked 'yes', and Section IX.7 was
checked 'no'. The division needs additional verification that the no high -priority sewer is
applicable for the request.
There are no high -priority lines associated with this project.
15A NCAC 02T .0107 Staff Review and Permit Preparation states:
• "(2) (e) If an application is accepted and later found to be incomplete, the applicant shall be
advised how the application or accompanying supporting information may be modified to make it
complete. The staff shall advise the applicant:
(2) if all required information is not submitted within 30 days, the project will be returned as
incomplete. Any resubmittal of a returned application shall be accompanied with a new application
fee."
