Dear Mr. Hughes,
Division of Water Resources’ Central and Regional staff have reviewed the application package
received on January 4, 2024. However, the Division requires additional information before completing our
review. Please address the items on the attached pages no later than the close of business on May 1, 2024.
Please be aware that the Applicant is responsible for meeting all requirements set forth in North
Carolina rules and regulations. The Applicant is also responsible for any oversights that occur during the
review of the subject application package. The Division may return the application as incomplete pursuant
to 15A NCAC 02T .0107(e)(2) if any omissions are made when responding to the outstanding items in
Sections A through V or the Applicant fails to provide the additional information on or before the above-
requested date.
Please reference the subject application number when providing the requested information. The
Applicant shall sign, seal, and date (where applicable) all revised and/or additional documentation and
submit an electronic response to my attention via the Non-Discharge online portal.

A. Overall:
1. The submitted application package is unclear about the total spray irrigation area for the proposed
facility. Item VI.8 of Form: RWNC 06-16 and portions of the Engineering Calculations list square
footage that totals 18.24 acres, while the Soil/Agronomist Evaluation, Water Balance,
Hydrogeologic Report, Project Narrative, O&M Plan, and portions of the Engineering Calculations
list either 16.97 acres or square footage that totals to 16.97 acres. Please verify the correct acreage
and revise the overall application package to reflect this.
2. Item VI.14 of Form: RWG 06-16 states that the Permittee will permit the influent pump station
separately. Is this referring to the collection system consisting of STEP tanks for the residences
and the swimming pool? If so, please remove all details and references to the collection system
from the Engineering Plans (Sheet D-101), Specifications, and Engineering Calculations. It is
permissible to leave references to the separate collection system in the Project Narrative and O&M
Plan since these describe the entire facility.
3. Item VII.2.e of Form: RWG 06-16 lists the number of five-day upset pond return pumps as two (2),
however, the Engineering Plans and Specifications depict one (1) return pump or a simplex five-
day upset pond return pump station. Per Item VI.11 of Form: RWG 06-16 and 15A NCAC 02U
.0402(h), multiple pumps shall be provided wherever pumps are used. Please either r evise the
application package to propose a duplex five-day upset pond return pump station or maintain a
spare uninstalled replacement return pump onsite that meets Alternative Design Criteria per 15A
NCAC 02T .0105(n).

1. Item VII.2.b – The table lists the number of units for the E-Z Treat Model 600 as one (1) in the
table, however, the Engineering Plans depict two (2). Please verify and revise.
2. Item VII.2.e –
a. The listed flow capacity of 90 gallons per minute (GPM) for the flow equalization tank pumps
does not match the Specifications, which list 80 GPM. Please verify and revise.
b. The listed flow capacity of 13.6 GPM at 46 feet of total dynamic head (TDH) for the calcite
filter pumps located in the wet weather storage pond dosing tank does not match the
Specifications, which list 30 GPM at 75 feet of TDH. Please verify and revise.
c. The listed flow capacity of 243 GPM at 297 feet of TDH for the field dosing pumps does not
match the Specifications, which list 261 GPM at 346.2 feet of TDH, or the Engineering
Calculations, which show the operating point in a different location. Please verify and revise.
d. The listed flow capacity of 40 GPM at 30 feet of TDH for the five-day upset pond return pump
does not match the Specifications, which list 32 GPM at 31 feet of TDH, or the Engineering
Calculations, which show the operating point matching the Specifications. Please verify and
revise.

2. The Licensed Soil Scientist (LSS) completed the fieldwork for the Soil & Agronomist Evaluation
more than one (1) year before the application package submittal. Per the Soil Scientist Evaluation
Policy, please include a statement from the LSS in the Soil & Agronomist Evaluation indicating
that the project site has not significantly changed since the original investigation.

L. Engineering Plans:
1. Overall –
a. Please include Sheet WW-101 (Existing Conditions) in the revised Engineering Plans.
b. Please provide a revised Sheet WW-102 (Overall Site Plan) that meets the Site Map
requirements per Instruction J of Form: RWG 06-16, Instruction J of Form: RWNC 06-16, and
15A NCAC 02U .0201(d).
c. The detail sheet page numbers are labeled “# of 13” but the sheet index on the title sheet only
goes up to Sheet D-112. Is there a Sheet D-113? Please verify and revise.
2. Sheet WW-113 –
a. Multiple leader lines appear to not point to the correct location in the northern portions of the
wastewater treatment facility and the five-day upset pond. Please verify and revise. This is
typical for all sheets (WW-117 and E-102.)
b. The linework for the influent 2” force main from the collection system is not shown. There is
a text label near the first surge tank that reads “Transition from 2” to 4” PVC 20’ from tank”
with two leaders. One of the leaders is pointing to the access road edge and the other to the 2”
PVC return force main from the five-day upset pond. Please verify and revise. This is typical
for all sheets (WW-114 and E-102.)
c. There is a text label for the 100-year floodplain boundary but no linework is shown. Please
revise. This is typical for all sheets (WW-117.)
d. There is a text label for the riparian buffer and the centerline of South Fork Withrow Creek but
no linework is shown. Please revise. This is typical for all sheets (WW-117.)
e. Please revise the “Creek Crossing” text label to point to the proposed horizontal directional
drilling (HDD) locations. This is typical for all sheets (WW-117).
f. Please show the potential well site buffer radius. This is typical for all sheets (WW-114 and
E-102.)
g. The text label for the spray head is not pointing to the correct location. Please revise. This is
typical for all sheets (WW-114, WW-115, E-102, and E-103.)
3. Sheet WW-117 – The text label for the 4” irrigation force main is not pointing to the correct
location. Please revise.
4. Sheet WW-119 – See Comment L.3.
5. Sheet D-104 –
a. The pipe invert labels in the wet weather storage pond dosing tank are switched for the five-
day upset pond and wet weather storage pond. Please verify and revise.
b. The listed effective storage volume of 362,056 gallons for the five-day upset pond does not
match the Engineering Calculations, which list 346,347 gallons. Please verify and revise.

6. Sheet D-106 – Detail 3 refers to an “Infiltration Basin”, which is not part of the proposed facility.
Please revise.
7. Sheet D-110 – Detail 1 for the chlorine/metering building depicts the 30-day liquid chlorine storage
area in front of the proposed eyewash station. Will the eyewash station be easily accessible if the
Permittee stores 30 days of 55-gallon liquid chlorine drums here?
8. Sheet D-112 – The finished floor elevation (FFE) of 354 feet called out in Detail 2 for the
chlorine/metering building does not match the rest of the Engineering Plans. Please verify and
revise.
9. Sheet E-101 – The text label specifies the auxiliary generator capacity as 150 kilowatts (kW) while
Item VII.2.h of Form: RWG 06-16 and the previous permit issuance list 20 kW. Please verify and
revise.
M. Specifications:
1. Overall –
a. Please add information for the two (2) E-Z Treat Model 600 calcite filters to the Specifications.
b. Please add a Safety Data Sheet (SDS) for the 12.5% sodium hypochlorite solution to the
Specifications.
2. Section 11307 (Ultraviolet Disinfection Units) – Part 1.01 specifies “a total of sixteen lamps (four
sets of four).” Is this quantity for three or four E-Z Treat Model 404 units? Please verify and
revise, if necessary.
3. Section 11401 (Effluent Filter Assembly) – Part 1.01 specifies a total of six (6) Polylok PL-525
effluent filters for the second surge tank. Should Part 1.01 include the one (1) Polylok PL-525
effluent filter shown in the wet weather storage dosing tank as well? Please verify and revise, if
necessary.
4. Section 11715 (Continuous Online Turbidimeter) – Please revise “Wastes Not Discharged to
Surface Waters (15A NCAC 02T .0100-.1600)” listed in Parts 1.01 and 1.02 to “Reclaimed Water
(15A NCAC 02U .0100-.1400)”.

N. Engineering Calculations:
1. Sizing/Storage Calculations – The chlorine contact/field dosing tank calculations use an 18,000-
gallon tank while Item VII.2.c of Form: RWG 06-16, the Project Narrative, and the Engineering
Plans specify a 30,000-gallon tank. Please verify and revise.
2. Treatment Calculations –
a. There appears to be a heading missing between the “Design Effluent Characteristics” and the
“Septic/Pump Tank Effluent.” Please verify and revise.
b. The note “Assumes 2/3 volume of…” is incomplete. Please revise for clarity.
c. Under “Media Filter and Dosing Tank(s) (E-Z Treat Pod),” the calculations show that the
design requires 98.39 pods but only 84 pods are provided. Please clarify this. Are the two (2)
E-Z Treat Model 600 calcite filters making up the difference?
3. Flotation Calculations – No comment.
4. Chlorine Dosing Calculations – See Comment N.1.
5. Wet Weather Storage Pond Calculations – No comment.
6. Five-Day Upset Pond Calculations – The table lists the pond bottom as 790 feet; however, Sheet
D-104 of the Engineering Plans depicts 788.5 feet as the pond bottom. Please verify and revise.
7. Irrigation Calculations – See Comment A.1.
8. Pump Calculations – Per Instruction F of Form: RWG 06-16 and 15A NCAC 02U .0201(c)(3),
please provide a friction/total dynamic head (TDH) calculation, system curve analysis, and pump
curve for the flow equalization pumps.

revise.
4. Under “Storage Pond Dosing Tank,” it states “The tank will house a total of four pumps…”
However, based on the list below, the total should be six (6). Please verify and revise.
5. Under “5-Day Upset Pond,” this section lists the effective storage volume of the five-day upset
pond as 362,056 gallons. See Comment Q.3.
6. Under “Chlorine Dosing Tank,” this section lists the tank size as 18,000 gallons. See Comment
N.1.

requires the Permittee to continuously monitor flow through the wastewater treatment facility.
The proposed facility does not have an influent flow meter included in the design. Please
demonstrate how the proposed facility will meet this condition. [15A NCAC 02T .0105(k),
