Highway 150 North to NCSR 2017 (Bethel Church Road) 2.7 miles North of Kernersville, N.C. Turn left onto NCSR 2017 and proceed 1. 1 miles to Glen Cross Road. Turn right on Glen Cross Road and proceed . 03 miles to 9330 Glen Cross Drive on the right just past the culvert. 6 . Discharge Point - Latitude: 36° 09 ' 38" Longitude: 80° 02 ' 48" Attach a USGS Map Extract and indicated treatment plant site and discharge point on map. USGS Quad No. C18NE or USGS Quad Name Belews Creek 7 . Size ( land available for expansion and upgrading) : 30,000 square feet. There is room for upgrading. 8 . Topography (relationship to flood plain included) : Slopes are 2. 6% to the west. 9. Location of nearest dwelling: There is a dwelling on the adjacent lot, approximately 250 ' to the east and upslope of the discharge point. 10 . Receiving stream or affected surface waters: Haw River a. Classification: 14,'S777' b. River Basin and Subbasin No. : Cape Fear ( 030601) c. Describe receiving stream features and pertinent downstream uses: The receiving stream is wide with a good flow. Downs tream uses are rural agricultural. Type of treatment (specify whether proposed or existing) : Existing standard septic tank subsurface sandfilter system with 391 sq. ft. of sand filter and chlorination. 5. Sludge handling and disposal scheme: Sludge is hauled by a licensed septic tank hauler to the nearest WWTP 6 . Treatment plant classification: N/A 7 . Special monitoring requests : N/A 3 . Additional effluent limits requests: N/A 4. Other: PART IV - EVALUATION AND RECOMMENDATIONS At the time of the inspection the system was operating well. No odor was present. Debris was in the stream at the discharge point, causing the water to pool. Ms . Cherry said she is in the process of cleaning it out. The husband inadvertently filled in the chlorination vent pipe. Holes were dug where Ms. Cherry had been trying to find the pipe. She said she will find it and keep chlorine tablets in it. I recommend the permit be reissued.
