This inspection was conducted to verify that the facility is operating in compliance with the conditions and limitations specified in NPDES WW Permit No. NCG550298. A summary of the findings and comments noted during the inspection are provided in the enclosed copy of the inspection report. The Compliance Evaluation inspection was conducted by Division of Water Resources staff from the Asheville Regional Office. Page#1 DocuSign Envelope ID: B173E14C-D35D-4C9D-852B-3C938CAC9E56 6/6/2024 6/7/2024 6/7/2024 NPDES yr/mo/day 24/05/30 Inspection Type C3111218 1 Section D: Summary of Finding/Comments (Attach additional sheets of narrative and checklists as necessary) Lauren Armeni and Mikal Willmer, with the Asheville Regional Office, conducted a Compliance Evaluation Inspection of the single-family wastewater disposal system that services 171 Utah Mountain Road on May 30, 2024. This inspection was conducted to determine if the system is being operated and maintained in compliance with General NPDES Permit No. NCG550000. Mr. Jeremy Messer (owner) was present and assisted in the inspection. The following items were noted during the inspection: 1. Operations & Maintenance: The previous inspection reports indicated that the system was failing. During this inspection, the system appeared to be functioning properly, and there was no visual impact to the receiving stream. 2. Effluent pipe: The effluent pipe was located, but was submerged under water. Since the pipe is submerged, it cannot be checked to determine if the system is discharging and if sampling is needed. 3. Effluent sampling: Mr. Jeremy Messer indicated that when the UV system was installed there was flow exiting the system, but sampling has not been conducted. A sampling port should be installed post-UV disinfection in order to sample and determine if there is flow since the effluent pipe is submerged. After the sampling port is installed, periodically check it to see if there is flow exiting the UV system. If at any time there is flow throughout the year, annual sampling needs to be conducted, per Table C.(1.) in General Permit NCG55, which will be included with this report. Does the facility analyze process control parameters, for ex: MLSS, MCRT, Settleable Solids, pH, DO, Sludge Judge, and other that are applicable? Previous reports indicated that the system was failing. At the time of the inspection, there was no impact to the receiving stream and no indication that the system was still failing. See summary for details. Comment: Septic Tank Yes No NA NE (If pumps are used) Is an audible and visual alarm operational? Is septic tank pumped on a schedule? Are pumps or syphons operating properly? Are high and low water alarms operating properly? One side of the tank is under the garage, but a riser was installed on the other side for access. The septic tank was last pumped on March 19, 2024 by Clearview Environmental Services. Are UV bulbs clean? Is UV intensity adequate? Is transmittance at or above designed level? Is there a backup system on site? Is effluent clear and free of solids? A Norweco UV disinfection system was installed in March 2024. There is a light on the side of the box that's green when it's working. The effluent from the UV system could not be evaluated because the box is enclosed and the effluent pipe was under water.
