REQUEST FOR MORE INFORMATION
Dear Mr. Pierce:
On November 2, 2007, the Division of Water Quality (DWQ) received your application dated September
28, 2007, to fill or otherwise impact 1.396 acres of 404/wetland (0.835 acres permanent impact and 0.561
acres temporary impact) and 606 linear feet stream impact (4461inear feet permanent impact, and 160
linear feet temporary impact), to construct the proposed residential subdivision. On October 10, 2007,
October 19, 2007, November 19, 2007, and November 29, 2007, the DWQ received additional
information from you, however, the DWQ has determined that your application remains incomplete
and/or provided inaccurate information as discussed below. The DWQ will require additional
information in order to process your application to impact protected wetlands and/or streams on the
subject property. Therefore, unless we receive five copies of the additional information requested below,
we will have to move toward denial of your application as required by 15A NCAC 2H .0506 and will
place this project on hold as incomplete until we receive this additional information. Please provide the
following information so that we may continue to review your project.
Additional Information Requested:
1. Please re-submit your site plans on full plan sheets at a scale of no smaller than 1 "=50'.
2. Please provide cross section details showing the provisions for aquatic life passage.
3. Please provide the location of any proposed stormwater management practices as required by
GC3705.
4. Please provide plans and profiles for all the stormwater management devices as required by
GC3705, including all required supplement forms and Inspection and Maintenance agreements.
5. Please indicate all stormwater inlets and outfalls on the site plan.

6. Please provide an inventory of proposed project imperviousness area based on the estimated
built-out conditions.
7. Please locate any planned sewer lines on the site plan.
8. Please locate all isolated or non-isolated wetlands, streams, and other waters of the State as
overlays on the site plan with topographic contours.
9. 4461inear feet of compensatory stream mitigation is required for this project. Please provide a
compensatory mitigation plan. The plan must conform to the requirements of 15A NCAC 2H
.0500 and must be appropriate to the type of impacts proposed.
10. Please explain why Impact No. 9 could not be avoided by relocation of the cul-de-sac road.
Please submit this information within 30 calendar days of the date of this letter. If we do not receive this
requested information within 30 calendar days of the date of this letter, your project will be withdrawn
and you will need to reapply with a new application and a new fee.
This letter only addresses the application review and does not authorize any impacts to wetlands, waters
or protected buffers. Please be aware that any impacts requested within your application are not
