Dlmyw Joshua and Jamie Mills 8013 Salix Drive Durham N.C. 27713 NORTH CAROLINA Enldhm mental Quality January 26, 2023 Subject: Compliance Evaluation Inspection Single Family Wastewater Treatment System General NPDES Permit NCG550000 Certificate of coverage NCG551527 Facility: 8013 Salix Drive Durham County Dear Mr. Mills, On December 20, 2023, Curtis Tyree from the Raleigh Regional Office visited your single- family residence (SFR) wastewater treatment system to evaluate compliance with the subject General NPDES Permit. Our records indicate the treatment system consists of a septic tank; a distribution box; a primary sand filter; a tablet chlorinator; a chlorine contact chamber; and a discharge pipe. General NPDES Permit NCG550000 and Certificate of Coverage (COC) NCG551527 authorize the discharge of domestic wastewater from your treatment system to an unnamed tributary to Northeast Creek, in the Cape Fear River Basin.. Findings during the inspection were as follows: I. The septic tank shall be checked annually and pumped out every 3 to 5 years. The septic tank was pumped out in February 2023. 2. Treatment system operation. The treatment system shall be maintained at all times to prevent seepage of sewage to the surface of the ground. At the time of the inspection, the system appeared to be well maintained. 3. Chlorination. The tablet chlorinator shall be inspected weekly to ensure there is an adequate supply of tablets for continuous and proper operation. Wastewater grade tablets (calcium hypochlorite) shall be added as needed to provide proper chlorination (swimming pool chlorine tablets shall not be used). At the time of the inspection, the chlorinator had a sufficient amount of tablets and correct type of tablets are being used.. A visual review of the outfall location shall be executed twice each year (one at the time of sampling) to ensure that no visible solids or other obvious evidence of system malfunctioning is observed Any visible signs of a malfunctioning system shall be documented, and steps taken to correct the problem. At the time of the inspection, the outfall location was clear and appeared to be well maintained and free of any obstructions. There was no water discharging at the time of the inspection. 5. Effluent sampling requirements. Effluent sampling must be conducted annually as part of your permit requirements. The effluent samples must be analyzed by a North Caroline Certified Lab and the results must be kept on site for three years. There were no effluent sampling results available at the time of inspection. G. Fees and renewals. COC's with unpaid administering and compliance monitoring fees will not be automatically renewed The fees must be paid annually and within 30 days of notification. All fees have been paid. The system appeared to be well maintained. The effluent pipe was a little difficult to find but the permittee knew where it was located and he will try to keep it maintained a little better.
