Division of Water Resources' Central and Regional staff have reviewed the application package
received on March 15, 2024. However, the Division requires additional information before completing our
review. Please address the items on the attached pages no later than the close of business on July 8, 2024.
Please be aware that the Applicant is responsible for meeting all requirements set forth in North
Carolina rules and regulations. The Applicant is also responsible for any oversights that occur during the
review of the subject application package. The Division may return the application as incomplete pursuant
to 15A NCAC 02T .0107(e)(2) if any omissions are made when responding to the outstanding items in
Sections A through M or the Applicant fails to provide the additional information on or before the above -
requested date.
Please reference the subject application number when providing the requested information. The
Applicant shall sign, seal, and date (where applicable) all revised and/or additional documentation and
submit an electronic response to my attention via the Non -Discharge online portal.

A. Overall:
1. The request to eliminate the setbacks for surface water diversions where NCDOT has control of
the stormwater inlets, pipes and channels is acknowledged. However, the setbacks for surface
waters are invariable and cannot be waived, regardless of any operation control process. A response
to this item is not required.
2. There have been two permit modifications to permit temporary earthen storage structures and
evaporation ponds for the storage and processing of diamond grinding slurry. One location in
Rowan County was permitted on September 10, 2019, and another was permitted on July 13, 2023.
Please provide an update on the status of these two storage ponds.
3. Permit Condition I.2. of the previous permit required an updated site map to be submitted with the
permit renewal application in accordance with 15A NCAC 02T .0105(d). Please provide updated
site plans for the temporary earthen storage structures.

E. Program Information:
1. This program also includes the operation of temporary earthen storage structures and evaporation
ponds permitted in the Attachment B Certifications approved on September 10, 2019 and July 13,
2023. The program information shall be updated to include the operation of these structures as a
part of this permit.
F. Operation & Maintenance Plan:
1. See Item E.1. The O&M Plan shall be updated to describe the operation of these earthen storage
structures in the program.
2. The program information for the hydrodemolition operation slurry and diamond grinding slurry
programs mention the use of mobile storage units as referenced in permit Condition I11.8. The O&M
Plan shall be updated to describe the operations of these storage structures for this program.
