Agreed. Looks pretty clear cut and no site visit needed on my end either. There are just a few items needed prior to
being able to issue the PJD:
1) Requesting party's signature on the PJD form. If the signature is from an agent versus NCDOT I will need an agent
authorization to go with it. Also, include the DOT contact name for who the PJD will be addressed.
2) Clarify on the NCSAM forms which one is for Lower Creek versus stream SA (and corresponding width/depth info).

Subject: JD Verification Request for U-5745, Caldwell County
Good Morning Gentlemen,
Axiom Environmental, Inc. (Axiom) has recently conducted a jurisdictional area delineation in association with planning
for U-5745, Intersection of US 64/NC 90/ NC 18 and US 64/NC 90 Improvements on the east side of Lenoir in Caldwell
County. This is an NCDOT Division 11 project. We have been requested to obtain agency verification of our delineation
and hope to obtain your assistance with this task.
Attached, you should find a zipped folder containing our preliminary JD package information. The folder includes the
following information:
A completed preliminary JD form
Project figures
Completed USACE and NCDWR stream forms and NC SAM forms
As you can see from the enclosed information, the project area includes no wetlands and two streams: perennial Lower
Creek and a perennial unnamed tributary to Lower Creek (Stream SA). A brief description of the stream features
follows:
* Lower Creek - This is a large, high-energy, incised, perennial stream that appears to have been relocated to one
side of the floodplain and straightened at the time of construction of Wilksboro Boulevard. This feature is bounded by a
multi-lane highway on one side and agricultural field on the other.
* Stream SA - This small, low-energy, perennial stream drains an apartment complex and mixed hardwood-pine
forest. The slow-moving water is characterized by much iron-oxidizing bacteria.
Again, we are interested in obtaining verification of ourjurisdictional area delineation. We found no wetlands and two
perennial streams. We will be happy to coordinate a site visit to view these features at your convenience. Please let us
know if you need additional information.
