Dear Mr. Lohr,
Division of Water Resources' Central and Regional staff have reviewed the application package
received on April 1, 2024. However, the Division requires additional information before completing our
review. Please address the items on the attached pages no later than the close of business on July 8, 2024.
Please be aware that the Applicant is responsible for meeting all requirements set forth in North
Carolina rules and regulations. The Applicant is also responsible for any oversights that occur during the
review of the subject application package. The Division may return the application as incomplete pursuant
to 15A NCAC 02T .0107(e)(2) if any omissions are made when responding to the outstanding items in
Sections A through R or the Applicant fails to provide the additional information on or before the above -
requested date.
Please reference the subject application number when providing the requested information. The
Applicant shall sign, seal, and date (where applicable) all revised and/or additional documentation and
submit an electronic response to my attention via the Non -Discharge online portal.

A. Overall:
1. The 8-inch effluent force main on parcel 747-13.75 containing the storage lagoon and spray fields
that are associated with this permit shall be permitted in this application rather than the
Summerhouse WWTP (Permit No. WQ0029945) because this modification is adding flow and a
new source to this facility. A response to this item is not needed.
2. Upon reviewing monitoring reports for this facility, it was noted that the Holly Ridge WWTF had
an average daily flow (ADF) of 214,876 gallons per day (GPD) in 2023, which exceed 90% of the
224,673 GPD permitted capacity for the disposal system. As this application is adding a new source
to this facility, the Permittee shall provide a demonstration that this modification does not violate
15A NCAC 02T .0118.

1. The signature authority in Item 1.3. Mr. Jeffrey T. Lohr is not listed in our system as an authorized
signature authority to sign on behalf of the Permittee. In accordance with 15A NCAC 02T .0106(a),
permit applications shall be signed by a principal executive officer of at least the level of vice-
president or other authorized employee. To meet permit application submission requirements, one
of the following actions shall be taken:
a. Provide an updated application form signed by an authorized permission pursuant to 02T
.0106(b).
b. Mr. Lohr shall be provided authority to sign permit applications in accordance with 15A NCAC
02T .0106(c). This delegation of authority shall be provided in writing to the Division by an
authorized person, in this case a principal executive officer of at least the level of vice-president
pursuant to 02T .0106(b). A copy of a signature authority delegation letter is provided with this
letter.

riprap erosion control structure at the connection point to the existing spray field storage pond, but
no detail was provided for it. A detail for this structure shall be provided.
