On March 12, 2024, the Division of Water Resources (Division) received your application requesting a 401 Water
Quality Certification form the Division for the subject property. The Division has determined that your application
is incomplete and cannot be processed. The application is on -hold until all of the following information is
received:
The application states that Impact S1 is for the "replacement of an existing culvert at a reconfigured angle
suitable to entrance drive". However, on the drawings you have provided, I'm not sure I see the location of
culvert replacement, but I see a culvert extension. Can you please elaborate. Please also provide
justification as to why this stream impact cannot be avoided by using the existing entrance road. — NCDOT
will not allow misaligned intersections, and so we have to align with Latta Ave (image below).

2. Please provide explanation of the Proposed Monument Sign in Stream SA. —This is vestigial and should be
revised per below.

Please provide a detailed engineering plan, profile view, and cross-section of all proposed culverts. These
drawings must include details regarding stream alignment in relation to pipe alignment, pipe slope, pipe
burial, and dissipater pad (riprap) if required for velocity control. If riprap is required, please enumerate,

and clearly label the temporary and permanent impacts on the site plan and submit a channel restoration
detail. Riprap must me located below the stream bed elevation. — I thought we provided a section orsome
information previously, but let us know what is needed on our side.
4. Please note that all work must be done "in the dry". Therefore, drawing methods will be necessary to
temporarily dewater the stream channel during pipe installation. Please provide a construction sequence
that details the method of dewatering to be utilized at the site and the sequence of construction evens to
be followed to ensure compliance with this condition. In addition, please ensure that the location of the
temporary impacts are shown on the construction drawings and clearly indicated in the revised impact
table. — I would think this would come from Carocon, but we can be part of the discussion to help
coordinate this.
