I am writingto you regarding the fast -track sewer extension application for M&H Storage (W00045474) Please receive this message as a formal request for additional information.

I have conducted a preliminary review of the application and supporting documents for the subject application. In accordance with 15A NCAC 02T.0107, please provide the information requested below:
The pump station is provided as a simplex station fora commercial building over 600 GPD flow. Simplex pump stations are considered alternative designs, and anew application must be submitted
to the Central Office (Raleigh) if this is the route you would like to go. If you wish this to remain in fast track, please revise the FTA so that two pumps are provided for the pump station.
2. Please provide a revised page 3 of the FTA where Section 6. Is filled out to denote what power reliability option the applicant wishes to opt for.
3. Please confirm that there are no pump stations between the proposed project and the downstream treatment facility
Per 15A NCAC 2T .0107(e) please submit this information within 30 days.
