Barry—the Certificate of Coverage [CoC] issued to your father in 2007 had no expiration date. It's still valid, and does
not need to be replaced.
I have attached a copy of the revised General Permit(NCG550000). That should be kept at the home. Any older
versions of it should be discarded.
After the house is sold,the new owners should complete and return the attached Name & Ownership Change form,
• along with a copy of the deed showing the date on which they took ownership. This cannot be done before the house is
sold, and is something the new owners must do.

Mr. Weaver,
Thank you so much for your return call and assistance. I hated to bother you but yours was the only name I could find
on the documentation I had maintained and, as stated, I knew it was dated from back in 2007. My parents are both in
their mid 80's now and have finally realized it is time to move to a smaller, more elderly friendly home.
I would appreciate a copy of the current Certificate of Coverage so that I may 1) have a record to give to my parents and
2) so I will know the next expiration date. You may reply to this email and attach the copy.
A final question —is there any special document(transfer form) we would need to provide to a prospective new
owner? I'm sure there must be some document(s) the new owner will need to submit along with a fee to transfer the
permit into their names if they are able to sell the home?
