With the passing on my husband and permit holder,Joseph D. Brummitt, I request to have the permit transferred to my name. Our son had inquired in 2014 about the process to transfer ownership of this permit as we had considered selling the residence at that time. He maintained an electronic copy of the Name Change form sent to him from 2014. I have completed this form and it too is enclosed.
