Dear Mr. &Mrs.Abbott:
Jenny Graznak of the Winston-Salem Regional Office performed a Compliance Evaluation Inspection
on the wastewater treatment system at your property in Forsyth County,North Carolina on January 30, 2009.
This type of inspection consists of two basic parts: a review of self-monitoring data, and an on-site inspection
of the facility and its discharge. You were present during the inspection.

You hold a General Permit to discharge Domestic Wastewaters from Single Family Residences under
the National Pollutant Discharge Elimination System(NPDES). The discharge from this system discharges to
an unnamed tributary to Muddy Creek. However,you stated that a discharge has never been determined at the
effluent pipe. If you notice any discharge in the future,please perform the effluent monitoring as listed in the
permit. Permit text Part I Section A (attached to this correspondence) specifies the limits and monitoring
requirements for your permit. The following parameters must be monitored on an annual basis: Flow,
Biochemical Oxygen Demand (BOD), Total Suspended Residue, and Fecal Coliform. A Laboratory with
North Carolina Wastewater Certification (a list of approved laboratories is attached) must perform this
monitoring. You must also conduct and document the following maintenance activities:
• Septic tanks shall be maintained at all times to prevent seepage of sewage
• Septic tanks will be checked at least yearly to determine if solids must be removed
• Septic tanks shall be pumped out within 3-5 years of issuance date of the Permit. Contents removed
from septic tanks shall be disposed of at a location and in a manner compliant with all local and state
regulations
The system is approximately 30 years old and does not have a chlorinator. The discharge pipe goes
underneath the driveway and front yard, emptying into a small creek. No discharge was observed during the
