We also have the responsibility to the public to safeguard these records and to carry out our day-today program obligations. DWQ file review and site inspection revealed that the site is now compliant with the terms and conditions of this permit. Wilmington, NC 28405-8037 Subject: NOTICE OF INSPECTION Frischkorn Site Permit No. SW8 990412 New Hanover County Dear Mr. Ward: On August 11, 2009, Steve Pusey of the Wilmington Regional Office of the Division of Water Quality (DWQ) performed a follow-up inspection of the Frischkorn site in New Hanover County to determine compliance with Stormwater Management Permit Number SW8 990412 issued on October 21, 1999. DWQ file review and site inspection revealed that the site is not compliant with the terms and conditions of this permit. Please find a copy of the completed form entitled "Compliance Inspection Report" attached to this letter, which summarizes the findings of the recent inspection. As indicated in the attached inspection report, the following deficiencies must be resolved: 1. Outlet swale does not have uniform 3:1 slope. This swale must be smoothed out to make the side slopes uniform. Swale must also be reseeded with grass or have grass sod installed in all bare areas. Also, please round out all sharp turns. 2. A submerged and vegetated perimeter shelf is required in the main pond. This shelf should have a slope of 6:1 or less, and vegetation must be established and maintained as per the O&M Agreement. 3.; Pond and forebay need to be mucked out by approximately 1 ft. depth, to match elevations shown in diagram of O&M Agreement. . 4. Eroded section of forebay berm must be filled in with dirt and riprap. 5. Forebay berm must be reseeded with grass or have grass sod installed in all bare areas (with the exception of the riprap area). 6. Please comply with all items in previous inspection report of May 28, 2009. Furthermore, a Plan Revision is required because the outlet swale was not built in accordance with the original plans. The revised plan should show the correct outlet pipe configuration and swale leading from the pipe outlet to the edge of the property. Please inform this Office in writing before August 28, 2009, of the specific actions that will be undertaken and the time frame that will be required to correct the deficiencies. Wilmington, NC 28405-8037 Subject: Notice of Inspection and Request for Additional Information Stormwater Permit No. SW8 990412 Frischkorn Site New Hanover County Dear Mr. Ward: On May 21, 2009, Steve Pusey of the Wilmington Regional Office of the Division of Water Quality (DWQ) inspected the Frischkorn Site in Brunswick County to determine compliance with Stormwater Management Permit Number SW8 990412 issued on October 21, 1999. DWQ file review and site inspection revealed that the site is not compliant with the terms and conditions of this permit. Please find a copy of the completed form entitled "Compliance Inspection Report" attached to this letter. In addition, this permit will expire on October 21, 2009. Section .1003(h) of 15 A NCAC 2H .1000 (the stormwater rules) requires that applications for permit renewals be submitted 180 days prior to the expiration date of a permit and must be accompanied by a processing fee, which is currently set at $505.00. Since we never received the permit renewal application or fee, please complete and submit the enclosed permit renewal application. Please reference the attached "Compliance Inspection Report" for details of actions needed to bring the system into compliance. In summary, the following checked information is needed to renew this permit: ® Permit Renewal Application. Original signatures must be on the permit renewal application. This office has never received the application. See form attached. ® Application Fee in the amount of $ 505. ® Designer's Certification. This office has not received a Designer's Certification for the Stormwater Management System associated with your facility. Systems that are certified on or after the date of this letter will need to have the original signature of the certifying person. See form attached. ® Maintenance items listed in the attached "Compliance Inspection Report." The requested information should be received by this Office prior to June 26, 2009. If you need additional time to submit the information, please mail or fax your request for a time extension to the Division at the address and fax number at the bottom of this letter. Remove leaves and yard debris located at the inlet rip-rap area to an 18" pipe routed under the driveway at the SE corner of the lot. 4. Remove leaves and yard debris located in and around the outlet structure. 5. Please explain the apparently clogged, approx. 10", concrete pipe going directly from the pond, in the vicinity of the outlet structure, to the discharge swale just north of the pond. This pipe is not shown on the plans. 6. Please fill out, sign, and return a Renewal Application form, SWU-102. This form has a page to verify the Operation & Maintenance Agreement. 7.
