Wet Detention Pond—CVS Pharmacy—Sneads Ferry Based upon a site visit conducted 5/4/09 it appears that all previously noted deficiencies have been addressed, although we do have the following comments regarding future maintenance. 1. All excessive vegetation in the vicinity of outlet structure has been removed. However, owner should make sure that remaining debris(floating dumps of killed cattails)does not dog outlet structure or draw down orifice. This debris may require periodic removal. 2. The day of the second inspection, the pond levels were still reduced from the recent maintenance, therefore vegetated shelves were exposed. Until such time that pond levels restore themselves to the design, permanent pool, the owner should make sure that these areas remain stable(non eroded). 3. All"woody vegetation"has been removed and disposed of. Owner should continue to remove these species(willow)as they appear. 4. Banks have been stabilized with sod. Owner should keep this area adequately watered to assure the sod property establishes. The report summarizes the findings of a recent inspection of the project's stormwater controls conducted on January 6, 2009 to determine compliance with Stormwater Permit Number SW8 981020 issued on March 10, 1999. The required actions that are necessary to bring this site into compliance are noted on the attached inspection report. Please inform this Office in writing before Monday, March 2, 2009, of the specific actions that will be undertaken and the time frame that will be required to correct the deficiencies. Failure to provide the requested information, when required, may initiate enforcement action including the assessment of civil penalties of up to $10,000 per day. As indicated in the attached inspection report the following issues must be resolved: 1) Submit a permit renewal form with the accompanying $505 fee. (SWU-101 with the "renewal" box checked) 2) Submit a completed "Operation and Maintenance" agreement form. 2) Have a professional engineer, usually the engineer who designed the system, certify that the pond has been installed and is operating as it was designed to, and submit this certification with the permit renewal package. If any deviations from the plan have occurred, please indicate them on the certification. 4) Stabilize the slopes and any other areas draining to the pond that are contributing excess sediment to the pond. The slope adjacent to the parking lot has been graded steeper than was indicated on the plans, and as a result some erosion has occurred. There is also some large shrubbery on the edges of the pond, but they don't appear to be destabilizing the bank in any way, and likely help to deter water fowl from using the pond. No evaluation was done to determine the sediment levels. It would be ideal if some of the cattails were removed since they take up some of the space that the sediment should occupy and encourage mosquito proliferation. This permit will be expiring on March 10, 2009, and the permit needs to be renewed in order to stay in compliance with the state stormwater regulations. In order to bring this site into compliance the following information must be received by the Division, and measures taken to maintain the pond properly. 1) Submit a permit renewal form with the accompanying $505 fee. (SWU-101 with the renewal box checked) 2)Submit a completed an "Operation and Maintenance" agreement form. 3) Have a professional engineer, usually the engineer who designed the system, certify that the pond has been installed and is operating as it was designed to, and submit this certification with the permit renewal package.
