Dear Mr. Couch:
On February 2, 2012, the Wilmington Regional Office received a plan revision for
Stormwater Management Permit Number SW8 980436. The revisions include
demolition and removal of the tennis court pad and various areas of the pool patio and
sidewalks and the construction of a new amenity building and associated sidewalk and
shelter area resulting in a net decrease of 557 square feet of impervious surfaces. It has
been determined that a formal permit modification is not required for the proposed
changes. We are forwarding you an approved copy of the revised plans for your files.
Please replace the old approved plan sheets with the new enclosed plan set.
Please be aware that all terms and conditions of the permit issued on April 28, 2009,
remain in full force and effect. Please also understand that the approval of this
revision to the approved plans for the subject State Stormwater Permit is done on a
case-by-case basis. Any other changes to this project must be approved through
this Office prior to construction. The issuance of this plan revision does not preclude
the permittee from complying with all other applicable statutes, rules, regulations or
ordinances which may have jurisdiction over the proposed activity, and obtaining a
permit or approval prior to construction.

II. PLAN REVISION INFORMATION

This flan revision entails renovations to the existing clubhouse and pool area;remove the existing tennis
court;and build a new amenity building in place of the existing tennis court.
III. SUBMITTAL REQUIREMENTS
Only complete application packages will be accepted and reviewed by the Division of Water Quality (DWQ).
A complete package includes all of the items listed below. The complete application package should be
submitted to the DWQ Office that issued the permit.

The Stormwater Management Permit No. SW8 980436 was renewed on April 28, 2009 and
effective until September 15, 2018. The purpose of this plan revision is to renovate the existing
clubhouse/pool area and build a new amenity building in place of the existing tennis court at the
Reserve at Forest Hills apartment community. This work entails removing the existing 7,234 sq.
ft. tennis court, build a new 2-story 7,198 sq. ft. amenity building (1'` floor footprint: 3,704 sq. ft.
and 2"' floor: 3,494 sq. ft.), and add 2,973 sq. ft. of impervious area associated with covered
porches, concrete sidewalk, and concrete pool deck improvements.
