Guidelines for Access: The staff of the Wilmington Regional Office is dedicated to making public
records in our custody readily available to the public for review and copying. We also have the
responsibility to the public to safeguard these records and to carry out our day-to-day program obligations.
Please read carefully the following guidelines before signing the form:
1. Please call at least a day in advance to schedule an appointment to review the files.
Appointments will be scheduled between 9:00 a.m. and 3:00 p.m. Viewing time
ends at 5:00 p.m. Anyone arriving without an appointment may view the files to the
extent that time and staff supervision is available.
2. You must specify files you want to review by facility name. The number of files that you
may review at one time will be limited to five.
3. You may make copies of a file when the copier is not in use by the staff and if time
permits. Copies of 25 or less are free. 26 or more require payment in full for all
copies made at 10 cents a copy: payment may be made by check, money order, or
cash at the reception desk. Copies can also be invoiced for your convenience.
4. FILES MUST BE KEPT IN THE ORDER YOU FOUND THEM. Files may not be taken
from the office. To remove, alter, deface, mutilate, or destroy material in one of these
files is a misdemeanor for which you can be fined up to $500.00.
5. In accordance with General Statute 25-3-512, a $20.00 processing fee will be charged
and collected for checks on which payment has been refused.

Please find attached a copy of the completed form entitled "Stormwater Compliance Inspection Report".
The report summarizes the findings of a recent inspection of the project's stormwater controls conducted on
April 27, 2000 to determine compliance with Stormwater Permit Number SW8 970705 issued to you on August
3, 1999.
A summary of the findings and comments are noted under Section 6 of the report. Please inform this
Office in writing before May 28, 2000 of the specific actions that will be undertaken and the time frame
required to correct the deficiencies. Failure to provide the requested information, when required, may initiate

STORMWATER COMPLIANCE INSPECTION REPORT
1. Project Name: Gourmet Center Subdivision
Project Number: SW8 970705 Control Measure: a wet detention pond
Water Body: Bradley Creek Classification: SC Sw HQW
Directions to site, address: Off New Centre Drive at South College Road, behind the carwash.
2. Inspection Date: April 27, 2000 .
Weather Conditions: sunny and cool
3. Compliance Issues:
a. The pond's vegetation is mostly dead.
b. As a result of the dead vegetation, the slopes have eroded and sloughed down onto the vegetated
shelf and into the pond, causing the pond's depth to be in question.
C. The sloughing slopes are in danger of covering the orifice.
d. A copy of the deed restrictions has not been sent to the Division.
4. Required Actions:
a. Please repair all eroded areas and replace the vegetation. You may need to provide an irrigation
system to make sure the vegetation gets established.
b. Clean out the sediment in the pond to the design depth, and reestablish the vegetated shelf.

1. A copy of the recorded deed restrictions has not been provided as required. This must be submitted at once.
2. There appeared to be an accumulation of sediment in the forebay. The levels should be checked and the area mucked out if
needed and returned to design specifications.
3. There were several areas where large woody vegetation has been allowed to grow on the vegetated shelf and sideslopes.
This should be cut.
4. There is an excessive amount of trash in and around the pond.
S. There are several heavily eroded areas on the sideslopes that should be repaired and reseeded.
6. The outlet structure did not appear to be level. This should be checked and returned to design status if required.
7. No operation and maintenance records were observed on the inspection date. If records are available, you may submit
copies along with your response to the non-compliance status of your pond.
