Dear Mr. Cox:
The following is a time line on compliance issues on the above referenced project:

1: 8/15/07 Received non-compliance status via ail
2: 8/16/07 Spoke with you at 3:40 pm to inform you that I would be out of town for
approximately two (2)weeks.
3: 9/05107 Called your office and spoke with Pat Durrett. I made her aware that i had
not received a renewal package.
4: 9/06107 Received new renewal package.
Ward Andrews(deceased) was the original engineer. I have contracted with
Sherwin Cribb to do an asbuilt drawing. Phil Tripp Vfth Tripp Engineering will do
an Engineer's Certification or whatever document is required for the permit renewal.
As soon as I know the completion dates, I will promptly notify you.

Dear Mr. Harris:
Please find attached a copy of the completed form entitled "Stormwater Compliance Inspection
Report". The report summarizes the findings of a recent inspection of the project's stormwater
controls conducted on 8/10/2007 to determine compliance with Stormwater Permit Number
SW8 970632 issued on 8/20/1997.
The required actions that are necessary to bring this site into compliance are noted under the
Comments section of the report. Please inform this Office in writing within fifteen (15) days of
the specific actions that will be undertaken and the time frame that will be required to correct
the deficiencies. Failure to provide the requested information, when required, may initiate
enforcement action. If you have any questions please contact David Cox at the Wilmington

Several items of concern were noted during the inspection that need your immediate attention. They are as follows:
1. There are some eroded inlet areas next to the outlet that are not shown on the plans. These could cause short circuiting.
2. There are some mild to moderate areas of erosion on the sideslopes that should be repaired.
3. There was NO water in the forebay at all. It was entirely grassed.
4. A pipe was installed through the berm which is not on the approved plans.
S. Due to the pipe installation in the berm, there was a eroded hole in the bottom of the permanent pool side of the pond.
6. Where is the rainfall runoff going since there is no water at all in the forebay?
7. There is no signed copy of the Engineer's certification or the recorded deed restrictions in the file. These need to be submitted
with your permit renewal request.
8_Large woody vegetation should be kept off of the pond slopes.
