Guidelines for Access: The staff of the Regional Office is dedicated to making public records in
our custody readily available to the public for review and copying. We also have the responsibility
to the public to safeguard these records and to carry out our day-to-day program obligations.
Please read carefully the following before signing the form.
1. Due to the large public demand for file access, we request that you call at least a day in
advance to schedule an appointment for file review so you can be accommodated.
Appointments are scheduled between 9:00 a.m. and 3:00 p.m. Viewing time ends at 4 45
p.m. Anyone arriving without an appointment may view the files to the extent that time and
staff supervision are available.
2. You.must specify files you want to review by facility name or incident number, as
appropriate. The number of files that you may review at one appointment will be limited to
five.
3. You may make copies of a file when the copier is not in use by the staff and if time permits.
There is no charge for 25 or less copies cost per copy after the initial 25 is 2.5 per copy.
Payment is to be made by check, money order, or cash in the administrative offices.
4. Files must be kept in the order you received them. Files may not be taken from the office.
No briefcases, large totes, etc. are Dermitted in the file review area To remove, alter,
deface, mutilate, or destroy material in public files is a misdemeanor for which you can be
fined up to $500.00.
5. In accordance with GS 25-3-512, a $25.00 processing fee will be charged and collected for
checks on which payment has been refused.
6. The customer must present a photo ID, sign-in, and receive a visitor sticker prior to
reviewing files.

Please find attached a copy ofthe completed form entitled"Stormwater Compliance Inspection Report".
The report summarizes the findings of a recent inspection of the project's stormwater controls conducted on
October 8, 1999 to determine compliance with Stormwater Permit Number S W.8 931222 issued on March 18,
1994.
A summary of the findings and comments are noted under Section 6 of the report. Please inform this
Office in writing before November 15, 1999 of the specific actions that will be undertaken and the time frame
required to correct the deficiencies. Failure to provide the requested information, when required, may initiate

2. Inspection Date: October 8, 1999 - Weather Conditions: sunny
3. Comparison of Approved plans vs. As-Built: Same
4. Compliance Issues:
a. No permanent vegetation established on pond bank.
b. There are eroded areas along the pond bank.
C. Trash and debris have accumulated around and within the pond.
d. There seems to have been.no regular maintenance done on the detention pond.
5. PE Certification Received? No Deed Restrictions Received? N/A
6. Required Actions:
a. Provide a permanent grass on the pond bank and keep mowed to below six inches in height.
b. Repair eroded areas and check pond for sediment accumulation. The original designed pond
depth was 4 foot. If the sediment has accumulated so that the pond has only a depth of 3 feet
or less than removal of the accumulated silt is required.
C. Check trash rack located at the outlet orifice for debris accumulation and clean any trash or
debris from the pond on a regular basis. Also clean debris from the outlet ditch from which the
pond drains.
d. A copy of the Operation and Maintenance Plan that you signed is provided. The maintenance
is expected to be done for the life of the project,which means as long as the impervious surface
is kept on site. If Wal-Mart Stores ever decides to sell the property than a permit ownership
form must be filled out and approved by this Office.
e. Please provide the Professional Engineer's Certification on the pond as required by the permit.
