Guidelines for Access: The staff of Wilmington Regional Office is dedicated to making public records in
our custody readily available to the public for review and copying. We also have the responsibility to the
public to safeguard these records and to carry out our day-to-day program obligations. Please read
carefully the following guidelines signing the form:
L Due to the large public demand for file access,we request that you call at least a day in
advance to schedule an appointment to review the files. Aj�pmtments will be scheduled
between 9:00am and 3:00ym. Viewing time ends at 4:45pm. Anyone arriving without an
appointment may view the files to the extent that time and staff supervision is available.
2. You must specify files you want to review by facility name. The number of files that you
may review at one time will-be limited to five.
3. You may make copies of a file when the copier is not in use by the staff and if time permits.
Cost per copy is$.05 cents.Payment may be made by check money order,or cash at the
reception desk Copies totaling$5 00 or more can be invoiced for Your convenience.
4. FILES MUST BE KEPT IN ORDER YOU FOUND THEM. Files may not be taken from
the office. To remove,alter,deface,mutilate,or destroy material in one of these files is a
misdemeanor for which you can be fined up to$500.00. No briefcases large totes,etc.are
Remitted in the file review area.
5. In accordance with General Statue 25-3-512,a$25.00 processing fee will be charged and
collected for checks on which payment has been refused.

Dear Mr. Hinkle:
Please find attached copies of the completed forms entitled "Stornwater Compliance Inspection
Re;.ort". The reports summarize the findings of a recent inspection of the Base's stormwater control
structures conducted on June 1, 1994 to determine compliance with Stormwater Certifications issued since
1988.
A summary of the findings and comments are noted under Section 6 of the report. Appropriate
action, if required, should be completed by July 2, 1994. Please inform this Office by letter when

STORMWATER COMPLIANCE INSPECTION REPORT
1. Project Name: P-804 Field Maintenance Complex
Project Number: 920215 Control Measure: 90 Detention
Water Body: Codgels Creek Classification: SC
Directions to site, address: Camp Lejeune, unsure of street name.
2. Inspection Date: June 1, 1994 -
Weather Conditions: Sunny, warm
3. Comparison of Approved plans vs. As-Built: Compared well.

6. Comments:
1. It was noted during the inspection that the orifice was submerged and no flow was
observed leaving the site. Upon review of the files, the entire outlet control system was
raised 2'. Based on the comment by Mr. Hinkle that a survey shot taken recently indicated
that the elevation of one corner of the concrete spillway was at 10. 10, the structure may
not have been raised by the full 2'. The centerline of the 5" orifice was approved at invert
8.5, approximately 2.5' below the lowest part of the spillway. I would judge that to be the
case at this pond, and it would appear that the pond is in conformance with the approved
plans. You may wish to consider the possibility of providing positive drainage from the
point the orifice becomes level with the concrete spillway down.to the edge of the spillway.
