C. SUBMITTAL REQUIREMENTS
Submit the application package to the appropriate DEMLR Regional Office (Coastal, SA Waters) or DEMLR
Central Office (Urbanizing Areas Ph 2, USMP, Non-Coastal HQW/ORW). Only applications packages that
include all required items listed below will be accepted and reviewed.
Initial each item below to indicate that the required information is provided in the application package:
ow- 1. A permit application processing fee of$505.00 payable to NCDEQ.
2. One original signed hard copy and one electronic copy of this completed form. The signing official
named on this application to represent the current permittee must meet one of the following:
a. Corpotation —a principle executive officer of at least the level of vice-president;
b. Limited Liability Company (LLC)—a manager or company official as those terms are defined
in G.S. 57D "North Carolina Limited Liability Company Act"
c. Public Entity—a principal executive officer, ranking official, or other duly authorized employee;
d. Partnership or limited partnership—the general partner;
e. Sale proprietor; or
f. Letter of authorization signed by one of the signatories noted in a—e above authorizing the
signature of another entity.
3. One hard copy and one electronic copy of recorded documents required by the original permit that
have not yet been received by DEMLR, including: deed restrictions, protective covenants,
condominium/planned community declaration and easements. If the project has been built, include
documentation that the maximum BUA per lot or maximum total BUA has not been exceeded. If
the project has not been built, include a signed agreement that the final recorded deed restrictions
and protective covenants will be submitted at a later date.

response to the first request for additional information, I will be logging this email correspondence as the second. Please
note that only two letters requesting additional information is allowed in the State Stormwater Program, this being the
second. Please pay special attention to the new due date,which is November 4`I', 2023, and in accordance with 15A
NCAC 02H. 1045(4)(a)(i). As noted in the letter, if all of the requested information cannot be provided by this due date,
the application will need to be returned in accordance with 15A NCAC 02H. 1045(4)(a)(ii)to allow for any additional time

Given that new legislation has recently passed, if the additional information is not provided by the new due date of
November 4`h, 2023,the$505 fee will be forfeited and the new fee is $750. Please refer to Section 12.14(a) of House Bill
259 for more information regarding these fees.

Effective October 3rd 2023, permit fees have increased as a result of recent legislation. For more information on the new

The Wilmington Regional Office received and accepted a State Stormwater Management Permit Renewal
Application for the subject project on August 4", 2023. A preliminary in-depth review of that information has
determined that the application is not complete, and that additional information is needed. To continue the
review, please provide the following additional information in a single, hard copy submittal package:
1. 15A NCAC 02H.1040(1)(a) and 1045(3)(a)(ii): The application was signed by Holly Mayberry; however,
the provided documentation from the NC Secretary of State does not identify Holly Mayberry as a vice
president or president of this corporation. 15A NCAC 02H.1040(1)(a): In the case of a corporation, the
application must be signed by a principal executive officer of at least the level of vice-president. Please
either:
a. Provide official documentation showing that Ms. Mayberry is a vice president or president and
authorized to sign for the corporation;
b. Have one of the identified officers of the corporation sign the application documents; or
c. The signature by other agents shall be accepted on the application only if accompanied by a letter
of authorization signed by the appropriate authority. Provide a letter of authorization from one of
the identified officers of this corporation authorizing Ms. Mayberry to sign on their behalf.
2. 15A NCAC 02H.1045(3)(a)(ii): Please confirm the following:
a. Section B.1 of the application lists the applicant as Southport Presbyterian Church. However,a
review of the North Carolina Secretary of State's (NC SOS) corporation database found
Southport Presbyterian Church of Presbyterian Church (U. S. A.). Please confirm that the
applicant listed on the application is consistent with the NC SOS's corporation database.
Please remember to confirm that any revised information is presented consistently throughout the application
documents. Please also remember to provide a hard copy of any updated documents per 15A NCAC
02H.1045(3).
The requested information should be received in this Office by September 21", 2023 in accordance with 15A
