Directions to Site: From Winston-Salem take US Highway 158 east to NCSR 2972 (Goodwill Church Road) . Turn right and proceed 0 . 8 miles to NCSR 1969 (Piney Grove Rd. ) . Turn right and proceed 1 . 1 miles to the entrance of Pine Knolls Development. Turn right and proceed to Lot 93, the second lot on the left. 6 . Discharge Point - Latitude: 36° 10 ' 59" Longitude: 80° 04 ' 19" Attach a USGS Map Extract and indicated treatment plant site and discharge point on map. USGS Quad No. C-18NE or USGS Quad Name Belews Creek 7 . Size ( land available for expansion and upgrading) : The size of the lot is approximately 27 , 000 feet. 8 . Topography (relationship to flood plain included) : Slopes are uniform and generally to the south at 2-6% . 9 . Location of nearest dwelling: The nearest dwelling is the residence at 1128 Pine Knoll Road located approximately 500 feet from the discharge site east. 10. Receiving stream or affected surface waters : Belews Creek a. Classification: "C" b. River Basin and Subbasin No. : Roanoke o3-oz=oi_ c. Describe receiving stream features and pertinent downstream uses : This area can best be described as rural residential . There are several other private dischargers within the area. Type of treatment (specify whether proposed or existing) : Existing 900 gallon septic tank, followed by a 391 square foot subsurface sand filter; followed by a disinfection system with 30 minute detentionwith 500 ft . n. " diameter outfall line . 5. Sludge handling and disposal scheme: Sludge is pumped by a licensed septic tank hauler and taken to the nearest WWTP for disposal . 6.
