Casey 8800 Buckey Court RECEIVED Clemmons, NC 27012 OCT 0 9 2015 Subject: Compliance Evaluation Inspection CENTRAL FILES Permittee: Alan E. Casey, Owner DWR SECTION Facility: 8800 Buckey Court, Clemmons, NC 27012 Permit: NCG550000 General Wastewater Permit Certificate of Coverage: NCG550083 Forsyth County Dear Mr. Casey: 1. Ron Boone, of the Winston Salem Regional Office (WSRO) of the NC Division of Water Resources (DWR or the Division), conducted a compliance evaluation inspection of the single family wastewater system at your place of residence on September 17, 2015. The assistance and cooperation of Julie Casey was greatly appreciated. An inspection checklist is attached for your records and inspection findings are detailed below. 2. The State requires the Division to inspect these types of systems every five years. Your system was last inspected in November, 2009. 3. The wastewater system at your residence appears to operating properly. Mr. Boone found no discrepancies or problems that indicated anything was wrong. However, the chlorinator and the end of the pipe could not be found, and therefore a full inspection could not be conducted. 4. At this point it is probably most prudent that you have the septic tank pumped at least every three to four years (with four people in the residence). Please keep records of septic tank pumping on hand as a historical record and proof that you had the tank pumped. The permit does require that you have the tank pumped as required. 5. Mrs. Casey was not aware of the effluent water ever having been tested by a laboratory. This is also a requirement of the permit, to have the effluent water tested once per year. However, because the effluent pipe could not be located during the inspection, it would be impossible to get the required samples anyway. It appears as though the effluent pipe was somehow covered with earth and that the effluent is simply seeping through the earth that covers the end of the pipe and it eventually seeps to the creek. It would be helpful, as far as getting samples and observing the effluent, if you could at some point uncover the end of the discharge pipe. 450 West Hanes Mill Road,Suite#300,Winston-Salem,North Carolina 27105 Phone:336-776-98001 Internet:www.ncdenr.gov An Equal Opportunity\Affirmative Action Employer—Made in part by recycled paper 6. The chlorinator also could not be located. It is unknown what type of chlorinator was originally installed at your home. Neither you nor the Division has any drawing or specifications of the system. 7. With the absence of such information, it is probably best for you to simply have the tank pumped as soon as possible, and then every three to four years, with four people in the residence. It is at this point impossible to apply chlorine and to have the water tested. 8. Please keep this letter with your records for the system along with copies of invoices from the Division for the annual permit fee, the Certificate of Coverage, records of septic tank pumping, maintenance and repair records, etc. 9. These types of systems should be inspected at least every five years to ensure they're properly operated and maintained. Failure to do so can result in expensive repair costs as well as failure of the system to properly treat the wastewater. Also, you need to have your septic tank pumped once every three to five years, depending on usage. 10. If you have any questions regarding this letter or the NCG550000 general permit, please don't hesitate to contact us at 336-776-9800. Sincerely, Aux, g 0 °f W.
